经支气管镜氩等离子凝固术联合冷冻治疗肺混合性鳞状细胞和腺性乳头状瘤1例并文献复习

摘　　要：肺混合性鳞状细胞和腺性乳头状瘤是一种罕见气道良性肿瘤,临床诊治经验少,其病因及发病机制尚无定论,临床表现无特异性,病理活体组织检查为诊断金标准,肿瘤切除为主要治疗方法。本文详细阐述1例因“发现左肺占位13 d”于贵州省人民医院住院男性患者的治疗过程。完善影像学、病理活体组织检查等检查后,明确诊断肺混合性鳞状细胞和腺性乳头状瘤,行支气管镜下氩等离子凝固术联合冷冻切除支气管腔内新生物,术后无特殊不适,复查胸部CT提示治疗效果良好出院,而后随访1年,无咳嗽不适。另外通过查阅文献,对该病的流行病学、临床诊断及治疗方式进行讨论总结。Pulmonary mixed squamous cell and glandular papilloma(PMSGP)is a rare benign tumor of airway.There is little experience in clinical diagnosis and treatment,the etiology and pathogenesis are still inconclusive,the clinical manifestations are not specific,pathological biopsy is the gold standard for diagnosis,and tumor resection is the main treatment.This article describes in detail the treatment process of a male patient who was hospitalized in Guizhou Provincial People′s Hospital for"finding left lung space occupying for 13 d".After improving imaging,pathological biopsy and other examinations,PMSGP were clearly diagnosed,and the bronchoscopic argon plasma coagulation combined with cryosurgery was performed to remove the new organisms in the bronchial lumen.There was no special discomfort after the operation.The chest CT reexamination showed that the therapeutic effect was good,and the patient was discharged from the hospital.After 1 year of follow-up,there was no cough and discomfort.In addition,the epidemiology,clinical diagnosis and treatment of the disease were discussed and summarized through literature review.
