摘  要:目的测定可溶性CD36(sCD36)在冠状动脉粥样硬化性心脏病(CHD)血浆中的变化,探讨其在CHD发生、发展中的作用。方法回顾性分析浙江省杭州市第三人民医院收治35例CHD患者资料作为CHD组,35名健康体检人员作为对照组,分析2组血浆sCD36、炎症指标[高敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6]、空腹血糖、血脂等指标,比较2组的差异,并以sCD36为因变量,以上述各项指标为自变量行线性相关与回归分析。结果CHD组三酰甘油(TG)、低密度脂蛋白胆固醇(LDLC)、血浆sCD36、hs-CRP、TNF-α、IL-6均高于对照组(P均<0.05),sCD36与hs-CRP、TNF-α、IL-6呈正相关(r=0.923,0.815,0.831,P均<0.05)。结论sCD36在CHD组升高,并伴有血脂代谢异常,sCD36可能为CHD的血浆标志物,通过炎症机制参与CHD的发生发展。Objective To determine the alteration in plasma soluble CD36(sCD36)in patients with coro-nary heart disease(CHD),and to explore its role in the development and progression of CHD.Methods A retro-spective data analysis was performed for CHD patients admitted to Hangzhou Third People's Hospital(patient group,n=35)and health checkup subjects in the hospital(control group,n=35).The two groups were compared for plasma levels of sCD36,inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor(TNF)-α,and interleukin(IL)-6],fasting blood glucose,and blood lipids.By using sCD36 as the dependent vari-able,a multiple linear regression analysis was performed with the indicators mentioned above as independent variables.Results The levels of triglycerides(TG),low density lipoprotein cholesterol(LDL-C),plasma sCD36,hs-CRP,TNF-α,and IL-6 were all higher in the CHD patient group than those in the control group(all P<0.05).The sCD36 level was positively correlated with the levels of hs-CRP,TNF-α,and IL-6(r=0.923,0.815,0.831,all P<0.05).Conclusion CHD patients usually have elevated level of sCD36 with concurrent dysfunctional lipid metabolism.sCD36 as a potential CHD marker may participate in the development of CHD via inflammatory mechanisms.
