海南省某医院六种恶性肿瘤患者住院费用的灰色关联分析

摘　　要：目的了解恶性肿瘤住院总费用、费用结构及灰色关联次序,为医院降低医疗费用,控制医疗费用增长提供依据。方法抽取海南省某医院2015年1月1日-2019年12月31日期间出院患者最多的六种恶性肿瘤患者的医疗费用1258例,利用excel 2007建立数据库并分析住院费用结构,采用灰色关联分析法对住院费用结构进行关联分析。结果六种恶性肿瘤患者平均住院费用31047.12元,西药费占住院费用的44.03%;平均实际补偿比70.58%;住院费用的灰色关联度由高到低分别是西药费(r_(3)=0.9893)、手术费(r_(8)=0.8003)、治疗费(r_(5)=0.7726)、化验费(r_(1)=0.7522)、放射费(r_(2)=0.7270)、床位费(r_(6)=0.7183)、检查费(r_(4)=0.7135)、其他费用(r_(9)=0.7075)、护理费(r_(7)=0.7026)。结论西药费用在恶性肿瘤患者住院费用中占比最大,医务人员劳动价值的费用没有得到有效体现,医院应当通过规范药品耗材使用、避免不合理检查和化验、缩短住院日等措施降低住院医疗费用。Objectives To understand the total cost,cost structure and grey correlation order of malignant tumor hospitalization,so as to provide basis for the hospital to reduce medical cost and control increase of medical cost.Methods The medical costs of 1258 patients with six kinds of malignant tumors who were discharged most from a hospital in Hainan Province from January 1st,2015 to December 31st,2019 were selected.Excel 2007 was used to establish a database and analyze the inpatient cost structure.Grey correlation analysis method was used to analyze the inpatient cost structure.Results The average hospitalization costs of patients with six kinds of malignant tumors were 31047.12 yuan,and western medicine costs accounted for 44.03%of the hospitalization costs.Average actual compensation ratio 70.58%;the grey correlation degree of hospitalization costs from high to low were western medicine cost(r_(3)=0.9893),operation cost(r_(8)=0.8003),treatment cost(r_(5)=0.7726),laboratory costs(r_(1)=0.7522),radiation costs(r_(2)=0.7270),bed costs(r_(6)=0.7183),examination costs(r_(4)=0.7135),other costs(r_(9)=0.7075),nursing costs(r_(7)=0.7026).Conclusions The cost of western medicine accounted for the largest proportion in the hospitalization cost of malignant tumor patients,and the costs reflecting the labor value of medical personnel had not been effectively reflected.Hospitals should reduce the hospitalization cost by standardizing the use of drug consumables,avoiding unreasonable examination and laboratory tests,shortening the stay time and other measures.
