柴胡清肝汤联合泼尼松治疗肉芽肿性乳腺炎临床疗效及其对免疫细胞功能的影响

摘　　要：目的 观察柴胡清肝汤联合泼尼松治疗肉芽肿性乳腺炎(GLM)临床疗效及其对免疫细胞功能的影响。方法 将我院108例GLM患者随机分为对照组与观察组各54例。对照组给予泼尼松治疗,观察组在对照组治疗基础上联合柴胡清肝汤治疗。比较两组临床疗效,乳房肿块大小、疼痛模拟评分法(VAS)、CD3^(+)、CD4^(+)、CD8^(+)、免疫球蛋白G(IgG)、IgA、IgM、补体CD3水平及疾病复发情况。结果 治疗后,观察组治疗总有效率显著高于对照组( P<0.05);两组乳房肿块大小、VAS评分显著低于治疗前,观察组乳房肿块大小、VAS评分显著低于对照组( P<0.05);两组CD3^(+)、CD4^(+)、补体CD3显著高于治疗前,CD8^(+)、IgG、IgA、IgM显著低于治疗前,观察组CD3^(+)、CD4^(+)、补体CD3显著高于对照组,CD8^(+)、IgG、IgA、IgM显著低于对照组( P<0.05);观察组复发率显著低于对照组( P<0.05)。结论 柴胡清肝汤联合泼尼松可明显提高治疗GLM患者疗效,调节免疫功能,避免疾病复发。Objective To observe the clinical efficacy of Chaihu Qinggan Decoction combined with prednisone in treatment of granulomatous mastitis(GLM)and its impact on immune cell function.Methods A total of 108 GLM patients were randomly divided into control group and observation group,with 54 cases in each group.Control group was treated with prednisone,while observation group was treated with Chaihu Qinggan Decotion on the basis of control group.The clinical efficacy,including breast mass size,pain simulation score(VAS),CD3^(+),CD4^(+),CD8^(+),immunoglobulin G(IgG),IgA,IgM,complement CD3levels,and disease recurrence were compared bwtween the two groups.Results After treatment,the total effective rate of observation group was significantly higher than that of control group( P<0.05);the size and VAS score of breast masses in both groups were significantly lower than before treatment,and were significantly lower in observation group than those in control group( P<0.05);the levels of CD3^(+),CD4^(+),and complement CD3 were significantly higher in both groups,and were significantly higher in observation group than those in control group( P<0.05);the levels of CD8^(+),IgG,IgA,and IgM were significantly lower in both groups than those before treatment,and were lower in observation group than those in control group( P<0.05);the recurrence rate in observation group was significantly lower than that in control group( P<0.05).Conclusion Chaihu Qinggan Decoction combined with prednisone significantly improves the efficacy on GLM by regulating immune function and avoiding disease recurrence.
