摘 要:目的 观察化滞柔肝颗粒治疗湿热中阻型非酒精性脂肪性肝病(NAFLD)临床疗效及其对肠道机械屏障的作用。方法 将我院湿热中阻型NAFLD患者90例随机分为化滞柔肝组与多烯磷脂酰胆碱胶囊组(易善复组)各45例,连续治疗3个月,比较两组治疗前后的血脂、肝功能及肠道机械屏障紧密连接蛋白封闭蛋白(Occludin)、闭锁小带蛋白-1(Zo-1)、连蛋白(Zonulin)等指标。结果 治疗后,两组总胆固醇(TC)、甘油三酯(TG)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)较治疗前下降,化滞柔肝组改善更为明显( P<0.05)。两组治疗后Occludin、Zo-1水平较前上升,Zonulin下降,化滞柔肝组改善程度优于易善复组( P<0.05)。结论 化滞柔肝颗粒治疗湿热中阻型NAFLD临床疗效确切,可改善血脂及肝功能,其机理可能与调控肠道机械屏障,改善肠屏障功能有关。Objective To observe clinical efficacy of Huazhi Rougan Granules on patients with dampness-heat accumulation type nonalcoholic fatty liver disease(NAFLD)and its effect on intestinal mechanical barrier.Methods Ninety patients with dampness-heat accumulation type NAFLD were randomly divided into Huazhi Rougan Granules group and Essentiale group,with 45 cases each.After 3 months treatment,blood lipids,liver function,intestinal mechanical barrier tight junction proteins Occludin,Zo-1,Zonulin were compared between the two groups.Results After treatment,total cholesterol(TC),triglyceride(TG),alanine aminotransferase(ALT)and aspartate amino tramsferase(AST)were decreased in both groups,and the improvements in Huazhi Rougan Granule group were more obvious( P<0.05);the levels of protein occludin and ZO-1 were increased,while zonulin decreased,and the degree of improvements in Huazhi Rougan Granules group were better than that in Essentiale group( P<0.05).Conclusion Huazhi Rougan Granules have a definite clinical effect in treatment of dampness-heat accumulation type NAFLD,which can improve blood lipid and liver function.Its mechanism may be related to the regulation of intestinal mechanical barrier and the improvement of intestinal barrier function.
