牵引旋转手法治疗椎动脉型颈椎病疗效的Meta分析

摘　　要：目的采用Meta分析评价牵引旋转手法治疗椎动脉型颈椎病的临床疗效。方法利用计算机检索知网、维普、万方、CMB、PubMed、The Cochrane Library、Web of science、Embase中有关牵引旋转手法治疗椎动脉型颈椎病的临床随机对照试验。由2名研究人员根据纳入和排除标准进行文献筛选,并对纳入文献进行资料提取和质量评价,采用RevMan5.2软件进行数据分析。结果纳入12项研究,共1781名患者。Meta分析结果显示:牵引旋转手法治疗椎动脉型颈椎病有良好的疗效,基于《中医病症诊断疗效标准》(OR=4.26,95%CI 1.94~9.36,P<0.001),基于症状体征积分(OR=5.13,95%CI 2.26~11.64,P<0.001);能够改善症状体征积分(MD=-3.40,95%CI-4.30~-2.50,P<0.001)及FS-CSA评分(SMD=-1.30,95%CI-2.18~-0.42,P=0.004)。结论牵引旋转手法能够改善椎动脉型颈椎病的临床症状。Objective:To systematically assess the effectiveness of rotation-pulling manipulation on patients withcervical spondylosis of vertebral artery type(CSA).Methods:Randomized controlled trials(RCTs)about rotation-pulling manipulation in the treatment of CSA were searched in CNKI,VIP,Wanfang,CBM,PubMed,The Cochrane Library,Web of science and Embaseby.the included data were extracted and appraised by two independent researchers according to the inclusion and exclusion criteria.Review Manager 5.2 was used to perform the meta-analyses.Results:12RCTs were included,involving 1781 patients.The results of meta-analyses showed that rotation-pulling manipulation had good treatment effect:based on Diagnostic Efficacy Criteria of TCM Syndromes(OR=4.26,95%CI 1.94~9.36,P<0.001),based on scores of symptoms and signs(OR=5.13,95%CI 2.26~11.64,P<0.001);This manipulation could improve scores of symptoms and signs(MD=−3.40,95%CI−4.30~−2.50,P<0.001)and scores of FS-CSA(SMD=−1.30,95%CI−2.18~−0.42,P<0.001).Conclusion:Rotation-pulling manipulation can improve the symptoms of CSA.
