摘　　要：目的研究体外受精-胚胎移植(IVF-ET)助孕期间精子生化指标对妊娠结局的影响。方法选取2019年7月至2022年3月邯郸市中心医院收治的139对接受IVF-ET助孕的夫妇作为研究对象。根据妊娠结局分为妊娠组(n=45)与未妊娠组(n=94)。检测两组男方精子形态、精子DNA碎片率与精浆锌水平,分析影响妊娠的因素,采用Pearson相关性分析精子生化指标与妊娠结局的相关性。结果妊娠组正常形态精子率、精浆锌水平高于未妊娠组,畸形精子率、精子DNA碎片率低于未妊娠组(P<0.05)。Logistic回归分析结果显示,正常形态精子率、畸形精子率、精子DNA碎片率与精浆锌水平是妊娠结局的影响因素(P<0.05)。Pearson相关性分析结果显示,正常形态精子率、精浆锌水平与妊娠率呈正相关(P<0.05),畸形精子率、精子DNA碎片率与妊娠率呈负相关(P<0.05)。结论IVF-ET助孕期间的精子形态、精子DNA碎片率、精浆锌水平是影响妊娠结局的重要因素,与妊娠结局密切相关,助孕期间应密切关注患者的精子状态。Objective To study the influence of sperm biochemical parameters on pregnancy outcome during in vitro fertilization-embryo transfer(IVF-ET).Methods A total of 139 couples receiving IVF-ET assisted pregnancy from July 2019 to March 2022 in Handan Central Hospital were selected as the research subjects.According to the pregnancy outcome,patients were divided into the pregnant group(n=45)and the non-pregnant group(n=94).Sperm morphology,sperm DNA fragmentation index,and seminal plasma zinc levels of two groups of male partners were detected,the factors affecting pregnancy were analyzed.Pearson correlation was used to analyze the correlation between sperm biochemical parameters and pregnancy outcomes.Results The proportion of normal sperm morphology and seminal plasma zinc levels in the pregnant group were higher than those in the non-pregnant group,while the abnormal sperm index and sperm DNA fragmentation index were lower than those in the non-pregnant group(P<0.05).The results of Logistic regression analysis showed that the proportion of normal sperm morphology,abnormal sperm index,sperm DNA fragmentation index,and seminal plasma zinc level were influencing factors for pregnancy outcome(P<0.05).The Pearson correlation analysis results showed that the proportion of normal sperm morphology,seminal plasma zinc level,and pregnancy rate were positively correlated(P<0.05);while the abnormal sperm index and sperm DNA fragmentation index were negatively correlated(P<0.05).Conclusions The sperm morphology,sperm DNA fragmentation index,and seminal plasma zinc levels during IVF-ET assisted pregnancy are important factors that affect pregnancy outcomes and are closely related to pregnancy outcomes.During assisted pregnancy,close attention should be paid to the patient′s sperm status.
