子宫内膜癌患者血清白介素-17、白介素-8、γ-干扰素水平与临床病理特征及预后的关系研究

摘　　要：目的 探究子宫内膜癌(EC)患者血清白介素-17(IL-17)、白介素-8(IL-8)、γ-干扰素(IFN-γ)水平与临床病理特征及预后的关系。方法 选取嘉兴市第二医院2017年12月至2020年12月收治的82例EC患者作为癌变组,选取同期就诊的65例子宫内膜良性病变患者作为良性病变组。检测患者入院时血清IL-17、IL-8及IFN-γ水平,分析上述指标与EC患者临床病理特征及预后的关系。结果 癌变组患者血清IL-17、IL-8水平高于良性病变组,IFN-γ水平低于良性病变组(P<0.05);与肌层浸润<1/2、无淋巴结转移EC患者比较,肌层浸润≥1/2、有淋巴结转移患者血清IL-17、IL-8水平升高,IFN-γ水平降低(P<0.05);IL-17、IL-8水平与EC患者国际妇产科联盟(FIGO)分期呈正相关(r=0.246、0.281,P<0.05),IFN-γ与FIGO分期呈负相关(r=-0.228,P<0.05);Cox回归分析结果显示,FIGOⅢ~Ⅳ期、IL-17高水平、IL-8高水平、IFN-γ低水平均是影响EC患者预后的危险因素。结论 IL-17、IL-8、IFN-γ异常表达可促进EC肿瘤细胞侵袭、迁移,通过检测EC患者血清IL-17、IL-8及IFN-γ水平可评估肿瘤进展情况,从而为临床诊疗提供指导。Objective To explore the relationship between serum levels of interleukin-17(IL-17),interleukin-8(IL-8),interferon-γ(IFN-γ)and clinicopathological characteristics and prognosis in patients with endometrial cancer(EC).Methods A total of 82 patients with EC who were treated in Jiaxing Second Hospital from December 2017 to December 2020 were selected as cancer group,and 65 patients with benign endometrial lesions in the hospital during the same period were included in benign lesion group.The levels of IL-17,IL-8 and IFN-γat admission were detected,and the relationship between the above serological indicators and the clinicopathological characteristics and prognosis of patients with EC were analyzed.Results The levels of serum IL-17 and IL-8 in cancer group were higher than those in benign lesion group while the level of serum IFN-γwas lower than that in benign lesion group(P<0.05).Compared with EC patients with muscular infiltration<1/2 and no lymph node metastasis,serum levels of IL-17 and IL-8 were increased while serum level of IFN-γwas decreased in patients with muscular infiltration≥1/2 and with lymph node metastasis(P<0.05).IL-17 and IL-8 levels were positively correlated with international federation of gynecology and obstetrics(FIGO)staging in patients with EC(r=0.246,0.281,P<0.05),and IFN-γwas negatively correlated with FIGO staging(r=-0.228,P<0.05).Cox regression analysis showed that FIGO stageⅢ-Ⅳ,high level of IL-17,high level of IL-8 and low level of IFN-γwere all risk factors affecting the prognosis of patients with EC.Conclusions Abnormal expression of IL-17,IL-8,and IFN-γpromote the invasion and migration of EC tumor cells.The detection of serum levels of IL-17,IL-8 and IFN-γin patients with EC can evaluate the progression of tumor,so as to provide guidance for clinical diagnosis and treatment.
