摘 要:目的探索在肺结核患者治疗过程中,分离出非结核分枝杆菌(nontuberculous mycobacteria,NTM)时与NTM肺病的相关性。方法通过回顾性调查分析2016年4月至2022年6月在首都医科大学附属北京胸科医院成功检出NTM的28例肺结核患者资料,追踪记录患者病例资料中临床、影像和微生物学资料,评估患者合并NTM肺病的可能性。结果28例患者分离出的NTM均为脓肿分枝杆菌。17例患者痰培养和/或分子生物学仅成功检测出1次脓肿分枝杆菌,8例患者在痰中分离到脓肿分枝杆菌2次以上,3例患者支气管肺泡灌洗液中培养和/或分子生物学检测发现脓肿分枝杆菌1次。参照NTM肺病诊断标准和随访结果,所有患者均不符合NTM肺病诊断标准。结论在肺结核患者治疗过程中分离出的NTM菌株一般不致病。Objective To exploretherelationship between non-tuberculousmycobacteria(NTM)isolated frompatients with pulmonary tuberculosis(TB)and NTM lung disease.Methods From April 2016 to June 2022,28 TB patients with NTM were retrospectively selected from Beijing Chest Hospital Affiliated to Capital Medical University Clinical data,imaging and microbiological data of patients were recorded and analyzed.ResultsThe isolated pathogen from all 28 patients was M.abscessus.17 patients detected M.abscessus once in sputum culture and/or molecular biology,while 8 patients detected M.abscessus more than twice in sputum.3 patients showed M.abscessus detection once in broncho-alveolar lavage fluid culture and/or molecular biology.According to the diagnostic and follow-up criteria for NTM lung disease,all patients did not meet the diagnostic criteria for NTM lung disease.Conclusion NTM strains dectection during the treatment of TB are generally not pathogenic.
