补肾通络汤合三棱莪术汤治疗膝骨关节炎肾虚血瘀证的临床疗效

摘　　要：目的 探讨补肾通络汤合三棱莪术汤治疗膝骨关节炎肾虚血瘀证的效果。方法 94例膝骨关节炎患者采用随机数字法分为对照组与试验组各47例。对照组口服洛索洛芬钠片和双醋瑞因胶囊治疗,试验组另给予补肾通络汤合三棱莪术汤治疗,连续治疗4周。评估两组患者临床疗效,比较两组患者中医症候积分、疼痛程度及骨关节炎指数、血清炎症因子、骨代谢及安全性。结果 试验组临床有效率高于对照组(P<0.05);与治疗后对照组比较,试验组中医症候积分、疼痛评分、WOMAC骨关节炎指数、白细胞介素-1β、肿瘤坏死因子-α、软骨寡聚基质蛋白降低,骨保护素、骨钙素升高(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 补肾通络汤合三棱莪术汤可显著提高肾虚血瘀证膝骨关节炎疗效,降低血清炎症因子水平,改善骨代谢,安全性好。Objective To investigate the clinical effect of Bu-shen-tong-luo decoction combined with San-leng-e-zhu decoction in the treatment of knee osteoarthritis(KOA)with syndrome of kidney deficiency and blood stasis.Ninety-four KOA patients were divided into a control group and an experimental group by random number table method,47 in each group.The control group was treated with loxoprofen sodium and diacedarineorally.The experimental group was treated with Bu-shen-tong-luo decoction combined with San-leng-e-zhu decoction.The treatment was continuous for 4 weeks.The clinical efficacy was evaluated.TCM symptom score,pain degree and osteoarthritis index,serum inflammatory factors,bone metabolism and safety were compared.The clinical effective rate of the experimental group was higher than that of the control group(P<0.05).Compared with the control group after treatment,TCM symptom score,pain score,WOMAC osteoarthritis index,interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)and cartilage oligomeric matrix protein(COMP)were reduced while osteoproglutinin(OPG)and osteocalcin(BGP)were increased(P<0.05).There was no significant difference in the adverse reactions between the two groups(P>0.05).Bu-shen-tong-luo decoction combined with San-leng-e-zhu decoction can significantly improve the efficacy of KOA with syndrome of kidney deficiency and blood stasis,reduce the level of serum inflammatory factors and improve the bone metabolism.The treatment has a good safety.
