摘 要:目的 分析细胞角蛋白19片段21-1(CYFRA21-1)、转移相关基因1(MTA1)、切除修复交叉互补基因1(ERCC1)检测在肺癌患者术前分期及淋巴结转移中的应用价值。方法 收集我院96例行手术治疗的非小细胞肺癌(NSCLC)患者临床资料,记录其癌组织与癌旁组织CYFRA21-1、MTA1、ERCC-1表达情况,比较三项指标在不同临床病理特征者癌组织中的表达差异,评估癌组织CYFRA21-1、MTA1、ERCC-1表达与NSCLC临床病理特征的关联性。结果 NSCLC患者癌组织CYFRA21-1、MTA1、ERCC-1表达阳性率均高于癌旁组织(P<0.05)。淋巴结转移者癌组织MTA1、ERCC-1阳性率及表达强度均高于未淋巴结转移者(P<0.05),TNM分期Ⅲ~Ⅳ期者癌组织CYFRA21-1、MTA1、ERCC-1阳性率及表达强度均高于Ⅰ~Ⅱ期者(P<0.05)。淋巴结转移与癌组织MTA1、ERCC-1表达强度呈正相关,TNM分期与癌组织CYFRA21-1、MTA1、ERCC-1表达强度呈正相关(P<0.05)。结论 检测癌组织CYFRA21-1、MTA1、ERCC-1表达情况,可辅助判断NSCLC临床分期及淋巴结转移,为临床治疗提供指导意见。Objective To analyze the application value of cytokeratin 19 fragment 21-1(CYFRA21-1),metastasis-associated gene 1(MTA1)and excision repair cross complement 1(ERCC1)in the preoperative staging and lymph node metastasis in patients with lung cancer.Clinical data of 96 patients with non-small cell lung cancer(NSCLC)who underwent surgical treatment in our hospital were collected.The expressions of CYFRA21-1,MTA1 and ERCC-1 in cancer tissues and adjacent tissues were recorded.The differences of their expressions in cancer tissues of patients with different clinicopathological characteristics were compared.The correlation of expressions of CYFRA21-1,MTA1 and ERCC-1 in cancer tissues with clinicopathological characteristics of NSCLC patients was evaluated.The positive expression rates of CYFRA21-1,MTA1 and ERCC-1 in the NSCLC tissues were higher than those in the adjacent tissues(P<0.05).The positive rates and expression intensities of MTA1 and ERCC-1 in patients with lymph node metastasis were higher than those in patients without lymph node metastasis(P<0.05).The positive rates and expression intensities of CYFRA21-1,MTA1 and ERCC-1 were higher in patients with TNM stage III-IV than those in patients with stage I-II(P<0.05).Lymph node metastasis was positively correlated with the expression intensities of MTA1 and ERCC-1 in cancer tissues.TNM staging was also positively correlated with expression intensities of CYFRA21-1,MTA1 and ERCC-1(P<0.05).The detection of CYFRA21-1,MTA1 and ERCC-1 in cancer tissues can help to determine the clinical staging and lymph node metastasis of NSCLC patients.It provides guidance for clinical treatment.
