摘　　要：目的:基于压力-应变环(PSL)定量评价丹黄通脉胶囊调控原发性高血压合并颈动脉粥样硬化斑块形成病人左心室心肌做功的临床价值。方法:将86例原发性高血压合并颈动脉粥样硬化斑块形成病人随机分为观察组(44例)与对照组(42例),两组病人均给予西医规范化降压、调脂方案,同时对照组口服丹黄通脉胶囊安慰剂,观察组口服丹黄通脉胶囊,采用常规超声检查两组病人左室质量指数(LVMI)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV);应用PSL技术对比分析治疗前后两组病人左心室做功状况,具体参数包括整体有用功(GCW)、整体无用功(GWW)、整体做功指数(GWI)、整体做功效率(GWE),同时对比两组间左室整体纵向应变(GLS)。结果:治疗后,两组LVMI、LVEDV、LVESV较治疗前均明显降低(P<0.05);观察组治疗后LVMI、LVEDV、LVESV均较对照组降低更为明显(P<0.05)。治疗后,两组GLS、GCW、GWW较治疗前均明显降低,GWI较治疗前明显升高,差异均有统计学意义(P<0.05);观察组治疗后GLS、GCW、GWW低于对照组,GWI高于对照组,差异均有统计学意义(P<0.05);两组治疗前后GWE无明显变化(P>0.05)。结论:丹黄通脉胶囊可有效降低高血压合并颈动脉粥样硬化斑块病人左心室心肌做功、抑制心室重塑。Objective:To quantitatively evaluate the clinical value of Danhuang Tongmai Capsule in regulating left ventricular myocardial work in patients with essential hypertension complicated with carotid atherosclerotic plaque formation based on pressure-strain loop(PSL).Methods:Eighty-six patients with essential hypertension complicated with carotid atherosclerotic plaque formation were randomly divided into observation group(44 cases)and control group(42 cases).Both groups were given standardized hypotensive and lipid regulation program of Western medicine.Meanwhile,Danhuang Tongmai Capsule placebo was taken orally in control group and Danhuang Tongmai capsule was taken orally in observation group.Left ventricular mass index(LVMI),left ventricular end diastolic volume(LVEDV),and left ventricular end systolic volume(LVESV)were compared between that two groups before and after treatment by conventional ultrasound examination.The PSL technology was used to compare and analyze the left ventricular work status of that two groups of patients before and after treatment,and the specific parameters included the global constructive work(GCW),global wasted work(GWW),global work index(GWI),global work efficiency(GWE).Then the overall global longitudinal strain(GLS)was compared between the two groups.Results:After treatment,LVMI,LVEDV,and LVESV in that two groups were significantly lower than those before treatment(P<0.05).LVMI,LVEDV,and LVESV in observation group were significantly lower than those in control group(P<0.05).After treatment,GLS,GCW,GWW and GWI in two groups were significantly decreased compared with before treatment(P<0.05).After treatment,GLS,GCW,GWW and GWI in observation group were significantly lower than those in control group(P<0.05).There were no significant changes in GWE in both groups before and after treatment(P>0.05).Conclusion:Danhuang Tongmai Capsule can effectively reduce left ventricular myocardial work and inhibit ventricular remodeling in patients with hypertension complicated with carotid atherosc
