基于文献分析胎盘滞留中医组方规律研究

摘　　要：目的:通过数据挖掘和文献分析探讨中医药治疗产后胎盘滞留的用药规律。方法:检索中国知网全文数据库、中国生物医学文献数据库、维普数据库2000年-2020年中医药治疗胎盘滞留的临床研究文献。确定纳入与剔除标准,建立数据库。对药物名称进行标准化处理,对药物性味、归经和功效类别的使用频次、频次分布进行统计和总结。结果:本研究纳入标准文献60篇,共涉及方剂61首,单味药82种。使用频次≥7的药物共18味,前十味药依次是当归、桃仁、川芎、益母草、甘草、炮姜、蒲黄、红花、五灵脂、黄芪。对使用频次≥2的中药进行分析,包括9类药物,51味中药,药物使用频次共555;药性使用频率最高的为温性,其次为平性、寒性;药味使用频率居前三位的依次是甘味、辛味和苦味;归经频率最高的是足厥阴肝经,其次是足太阴脾经和手少阴心经;按照功效分类,使用频率居前五位的药物依次是活血化瘀药、补虚药、止血药、清热药和理气药。结论:中药治疗胎盘滞留多以活血化瘀、补气益血为法,同时注意及时止血止痛,临床多以生化汤为基础进行加减。Objective:To explore the medication rules of TCM prescriptions in treating postpartum placenta retention by data mining and literature analysis.Methods:The clinical literature of treating placenta retention in TCM from 2000 to 2020 was retrieve in CNKI,SinoMed and VIP databases.The inclusion and exclusion criteria was determined to establish a database.The name of the medicine was standardized,and the medicinal property,medicinal taste,meridian tropism,and efficacy categories of the included medicines were counted.Results:Sixty studies met the criteria were included in this study,including 61 prescriptions and 83 single medicines.There were 18 medicines with frequency of no less than 7 times,and the top 10 medicines were Danggui(Radix Angelicae Sinensis),Taoren(Semen Persicae),Chuanxiong(Rhizoma Chuanxiong),Yimucao(Herba Leonuri),Gancao(Radix Glycyrrhizae),Paojiang(Rhizoma Zingiberis Preparatum),Puhuang(Pollen Typnae),Honghua(Flos Carthami),Wulingzhi(Faecas Trogopterori)and Huangqi(Radix Astragali).TCM medicines with a frequency of not less than 2 times were analyzed,including 9 kinds of medicines and 51 TCM medicines,and the total frequency of medicine used was 555 times.The highest frequency of medicine used was warm medicine,followed by the frequencies of moderate and cold medicine.The top three medicinal tastes were sweet,pungent and bitter,respectively.The highest frequency of meridian tropism was liver meridian of foot-Jueyin(足厥阴),followed by the frequencies of medicines flowed into spleen meridian of foot-Taiyin(足太阴)and heart meridian of hand-Shaoyin(手少阴).According to the classification of efficacy,the top five medicines frequently used were blood activating and stasis removing medicines,tonifying medicines,hemostatic medicines,heatclearing medicines,and Qi(气)-regulating medicines.Conclusion:The clinical treatment of placenta retention in TCM is mainly based on the method of promoting blood circulation to remove blood stasis,and replenishing Qi and blood,while paying attention to hemost
