李晶主任运用五合止痛方治疗胃痛的经验

摘　　要：胃痛是消化内科的一种常见病、多发病,临床表现为胃脘近心窝处的疼痛。山西中医药大学李晶主任通过总结多年临床经验,认为胃痛病位在胃、肝、脾,多因为饮食、情志、气血不调等致病。李晶主任将本病机分为三类:一则以脾虚气滞,气机失调为主;二则以虚实夹杂、寒热错杂并举;三则久病不解,虚、瘀、毒并见;并提出治以益气健脾温中、行气化瘀解毒为主的治疗思想。李晶主任根据治疗思想创制了五合止痛方,是融合了枳术丸、百合乌药汤、小建中汤、金铃子散、丹参饮,并随证加减,全方共奏行滞消胀、益气健脾、理气止痛、活血化瘀、解郁调中之功。文章分析一例案例,介绍李晶主任五合止痛方治疗胃痛的经验,通过分析用药过程及其机制,体现李晶主任治疗胃痛的学术思想,患者疗效显著,以期为临床参考。Stomachache is a common and frequently occurring disease in the department of gastroenterology.Its clinical manifestation is epigastric pain.LI Jing,director of Shanxi University of Traditional Chinese Medicine,summarized many years of clinical experience,and believed that stomach pain was located in the stomach,liver,and spleen,and was mostly caused by diet,emotion,Qi(气)and blood disorders,etc..He divided the pathogenesis into three categories,First is mainly about spleen deficiency and Qi stagnation,Qi imbalance.Second is mainly about deficiency-excess mixing,cold-heat complex.The third is mainly about chronic illness,deficiency,blood stasis and toxin.He also puts forward the therapeutic idea of supplementing Qi,strengthening spleen,warming the middle,promoting Qi,removing blood stasis and detoxifying.Director LI Jing created the Wuhe Zhitong decoction(五合止痛方)on his therapeutic ideas,which integrates Zhizhu Wan(枳术丸),the Baihe Wuyao decoction(百合乌药汤),the Xiaojianzhong decoction(小建中汤),Jinlingzi San(金铃子散),and Danshen Yin(丹参饮),and adds or subtracts them according to the syndrome.The entire formula plays the role of promoting stagnation and bloating,tonifying Qi and spleen,regulating Qi and pain,promoting blood circulation and resolving stasis,and regulating depression.The article analyzes a case study and introduces the Director LI Jing’s experience the Wuhe Zhitong decoction in treating stomach pain.By analyzing the medication process and mechanism,it reflects Director LI Jing's academic philosophy of treating stomach pain.The patient's efficacy is significant,and it is expected to serve as a clinical reference.
