马小军治疗2型糖尿病合并高脂血症经验总结

摘　　要：糖尿病是一种代谢性疾病,由于遗传或环境等多种因素所致胰岛素分泌、胰岛素作用或两者兼有缺陷,造成的碳水化合物、脂肪和蛋白质代谢异常的一组以慢性血葡萄糖水平增高为特征的代谢综合征。2型糖尿病是最常见的内分泌疾病。由于患者的不良饮食结构及生活方式,2型糖尿病患者中普遍存在高脂血症,如不及时干预或引起冠状动脉粥样硬化性心脏病、脑卒中等心脑血管疾病。目前治疗此病药物局限且不良反应居多。中医药治疗为治疗此病开拓新思路。甘肃天水市中医医院马小军主任认为本病根本为“痰湿、内热”补虚泻实。马主任提出以“健脾化痰,补肾养阴”之法贯穿治疗始终,对脾虚痰阻型以导痰汤治疗;痰瘀互阻型以二陈汤合血府逐瘀汤治疗;肝郁脾虚型以逍遥散治疗;均取得显著疗效。文章结合案例,介绍马小军主任治疗2型糖尿病合并高脂血症的临床经验,以期为临床参考。Diabetes mellitus is a metabolic disorder characterized by chronic elevated blood glucose levels due to defects in insulin secretion,insulin action,or both due to genetic or environmental factors,resulting in abnormal carbohydrate,fat and protein metabolism.Type 2 diabetes is the most common endocrine disease.Due to the poor diet structure and lifestyle of patients,hyperlipidemia is common in type 2 diabetes patients.If they cannot be treated in time or cause coronary heart disease,stroke,cardiovascular and cerebrovascular diseases.At present,the treatment of this disease is limited by drugs,and there are many adverse reactions.Traditional Chinese medicine treatment opens up new ideas for the treatment of this disease.Ma Xiaojun,director of Gansu Tianshui Traditional Chinese Medicine Hospital,believes that the disease is basically phlegm dampness and internal heat.Director Ma proposed to use the method of invigorating the spleen and removing phlegm,tonifying the kidney and nourishing yin(阴)throughout the treatment.For the type of spleen deficiency and phlegm obstruction,we should treat it with the Daotan decoction(导痰汤).For the type of phlegm and blood stasis we should treat it with the Erchen decoction(二陈汤)and Xuefu Zhuyu decoction(血府逐瘀汤).For the type of liver depression and spleen deficiency type we should treat it with Xiaoyao powder(逍遥散).This article introduces the clinical experience of Director Ma Xiaojun in treating type 2 diabetes with hyperlipidemia,with a view to providing clinical reference.
