摘 要:脂阿拉伯甘露聚糖(lipoarabinomannan,LAM)是结核分枝杆菌(Mycobacterium tuberculosis)细胞壁的组成成分,是一种糖脂,可使巨噬细胞失活,并清除氧化自由基。LAM存在于结核病患者的尿液、血清和肺部,因此人们开发了以LAM作为生物标志物的床旁检测(point-of-care-testing,POCT)方法,以减少诊断延迟,及时采取相应干预治疗措施阻断结核病传播。与检测单纯结核病患者相比,LAM检测技术对于诊断晚期免疫缺陷和CD4细胞计数低的艾滋病病毒(HIV)感染相关结核病患者具有更好的敏感性,对于这部分患者以及无法产生痰液的儿童、重症结核病患者等具有较好的应用价值。本文通过介绍LAM的结构、功能及其在结核病诊断中的应用研究进展,以期为未来结核病防控提供参考和思路。Lipoarabinomannan(LAM),as a glycolipid,is a component of the cell wall of Mycobacterium tuberculosis(Mtb),can inactivate macrophages and remove oxidizing free radicals.As LAM is available in the urine,serum and lungs of TB patients,point-of-care-testing(POCT)taking LAM as a biomarker has been developed to reduce any delay in diagnosis and to timely take appropriate intervention and treatment measures to block TB transmission.Compared with single tuberculosis patients,LAM is sensitive to relevant TB patients infected by HIV with low count of CD4 cells and immunodeficiency at late period of diagnosis,indicating a good application value for these patients,children who could not produce sputum,and TB patients with severe symptoms.In conclusion,the structure,function and application of LAM in the diagnosis of TB was introduced,with a view to providing a reference or an idea for future prevention and control of TB.
