清肺凉血方联合双波长强脉冲光治疗女性肺经风热型玫瑰痤疮的临床疗效及其作用机制的初步研究

摘　　要：目的:观察清肺凉血方联合双波长强脉冲光治疗女性肺经风热型玫瑰痤疮的临床疗效,并对其可能的机制进行初步研究。方法:2020年5月—2020年12月治疗玫瑰痤疮女性患者105例,按照随机数字表法分为强脉冲光组、中药组和联合组,每组35例。强脉冲光组治疗第0周、第2周、第4周、第6周、第8周进行双波长强脉冲光照射治疗,中药组服用清肺凉血方,联合组在强脉冲光组治疗的基础上服用清肺凉血方;疗程为8周。采用玫瑰痤疮观察指标评分表评价3组患者的治疗效果,治疗前后采用酶联免疫吸附法检测血清IL-17和IL-37含量。结果:联合组总有效率为88.5%,高于强脉冲光组的71.4%和清肺凉血方组的74.3%,差异有统计学意义(P<0.05)。治疗后,联合组的红斑、毛细血管扩张、灼热、干燥等症状体征评分均低于治疗前,差异具有统计学意义(P<0.05);联合组的红斑及毛细血管扩张评分低于中药组,而灼热及干燥评分低于强脉冲光组,差异具有统计学意义(P<0.05)。联合组血清IL-17和IL-37水平较治疗前下降,差异具有统计学意义(P<0.05),联合组治疗后血清IL-17和IL-37水平低于强脉冲光组,差异具有统计学意义(P<0.05)。结论:清肺凉血方联合双波长强脉冲光可以有效改善玫瑰痤疮患者的临床症状体征,其作用机制可能与下调IL-17及IL-37水平有关。Objective:To evaluate the clinical efficacy of Qingfei Liangxue Decoction combined with dual wavelength intense pulsed light in the treatment of female rosacea of lung meridian wind heat syndrome and to explore its possible mechanism.Methods:105 female patients with rosacea were selected from January5,2020 to December 31,2020 They were randomly divided into intense pulsed light group,Chinese medicine group and combined group,with 35 cases in each group.The intense pulsed light group received dual wavelength intense pulsed light irradiation at week 0,week 2,week 4,week 6 and week 8,the Chinese medicine group took Qingfei Liangxue Decoction,combined group was treated with intense pulsed ligh combined with Qingfei Liangxue Decoction.The course of treatment was 8 weeks.The therapeutic effect of patients was evaluated by rosacea observation index score table,and the symptoms and signs score of the three group patients were evaluated before and after treatment,the serum IL-17 and IL-37 levels were detected by ELISA.Results:The effective rate of the combined group was 88.5%,which was better than 71.4%and 74.3%in the intense pulsed light group and the Chinese medicine group(P<0.05).After treatment,the scores of erythema,telangiectasia,burning and dryness in the combined group were lower than those before treatment(P<0.05),the scores of erythema and telangiectasia in the combined group were lower than those in the Chinese medicine group,while the scores of burning and drying were lower than those in the intense pulsed light group(P<0.05).The levels of serum IL-17 and IL-37 in the combined group were significantly lower than those before treatment(P<0.05),After treatment,the serum levels of IL-17 and IL-37 in the combination group were lower than those in the strong pulsed light group,and the difference was statistically significant(P<0.05).Conclusion:Qingfei Liangxue Decoction combined with dual wavelength intense pulsed light can effectively improve the clinical symptoms and signs of patients with rosacea,and its mechani
