银离子敷料治疗皮肤深Ⅱ度烧烫伤创面的有效性及安全性观察

摘　　要：目的探讨银离子敷料治疗皮肤深Ⅱ度烧烫伤创面的有效性及安全性。方法自2019年10月至2020年6月,南宁市第二人民医院烧伤整形外科对132例皮肤深Ⅱ度烧烫伤创面患者进行治疗,根据治疗方法分为治疗组(65例)和对照组(67例),治疗组采用银离子敷料,对照组采用含银敷料Atrauman Ag。比较两组患者的总有效率、创面愈合率、创面愈合时间、创面分泌物细菌培养转阴率,并检测患者治疗前后的生命体征、实验室检查、心电图、综合评价及血银、尿银含量等,并记录治疗期间不良反应事件。结果基于全分析集(full analysis set,FAS)和符合方案集(per protocol set,PPS),结果均显示治疗组的总有效率高于对照组(P<0.05)。两组的创面愈合率、创面愈合时间,差异均无统计学意义(P>0.05)。两组治疗前后创面分泌物细菌培养转阴率比较,差异无统计学意义(P>0.05)。患者在治疗前后的生命体征、实验室检查、心电图、综合评价及两组间血银、尿银浓度比较,差异均无统计学意义(P>0.05)。两组均未发生严重不良反应事件。结论银离子敷料的有效性不劣于含银敷料Atrauman Ag。治疗期间两组均未发生严重不良反应事件,因此该银离子敷料是一种安全、有效的治疗皮肤深Ⅱ度烧烫伤创面的敷料。Objective To explore the effectiveness and safety of silver ion dressings in treating deep second-degree burns and scalds.Methods From October 2019 to June 2020,Department of Burn and Plastic Surgery,the Second Nanning People's Hospital treated 132 patients with deep second degree burn and scald wounds.Divided into a treatment group(65 cases)and a control group(67 cases),the treatment group was treated with silver ion dressing,while the control group was treated with silver containing dressing Atrauman Ag.Compare the total effective rate,wound healing rate,wound healing time,and negative rate of bacterial culture of wound secretions between the two groups of patients,and detect the patient's vital signs before and after treatment,laboratory examinations,electrocardiogram,comprehensive evaluation,and silver content in blood and urine,etc.Record serious adverse reactions events during treatment.Results Based on the full analysis set(FAS)and per protocol set(PPS),the results show that the total effective rate of the treatment group is not inferior to that of the control group(P<0.05).There was no statistical difference in wound healing rate and wound healing time between the treatment group and the control group(P>0.05).There was no significant difference in the rate of bacterial culture of wound secretions turning negative between the two groups before and after treatment(P>0.05).There was no significant difference in the subjects'vital signs,laboratory examinations,electrocardiogram,comprehensive evaluation,and blood silver,urine silver concentration between the treatment group and the control group before and after treatment(P>0.05).There were no serious adverse reactions events in the experimental group and the control group.Conclusion The effectiveness of silver ion dressing is not inferior to silver dressing Atrauman Ag.During the treatment,no serious adverse reactions events occurred in the two groups.Therefore,the silver ion antibacterial functional dressing is a safe and effective dressing for deep second-d
