阶梯式康复训练结合音乐疗法在腰椎间盘突出症术后患者中的应用

摘　　要：目的探讨阶梯式康复训练结合音乐疗法在腰椎间盘突出症术后患者中的应用效果。方法采用便利抽样法,选取2019年4月—2021年4月郑州人民医院收治的176例腰椎间盘突出症术后患者为研究对象,其中2019年4月—2020年4月收治的81例患者为对照组,2020年5月—2021年4月收治的95例患者为观察组。对照组给予常规护理,观察组在对照组基础上实施阶梯式康复训练结合音乐疗法。术后1 d、14 d、1个月、3个月、6个月,比较两组抑郁、焦虑、疼痛及功能障碍指数量表评分。结果术后1、3、6个月,观察组抑郁、焦虑评分低于对照组,差异均有统计学意义(P<0.05)。重复测量方差分析结果显示,两组抑郁、焦虑评分比较存在组间、时间及交互效应,差异均有统计学意义(P<0.05)。术后14 d、1个月、3个月、6个月,观察组疼痛、功能障碍指数量表评分低于对照组,差异有统计学意义(P<0.05)。重复测量方差分析结果显示,两组疼痛、功能障碍指数量表评分比较存在组间、时间及交互效应,差异均有统计学意义(P<0.05)。结论阶梯式康复训练结合音乐疗法可缓解腰椎间盘突出症术后患者的负性情绪,减轻术后疼痛,提高康复效果,值得临床推广应用。Objective To explore the effect of stepwise rehabilitation training combined with music therapy in postoperative patients with lumbar disc herniation.Methods From April 2019 to April 2021,a total of 176 postoperative patients with lumbar disc herniation admitted to People's Hospital of Zhengzhou were selected as study subjects using convenience sampling method,81 patients admitted from April 2019 to April 2020 were set as the control group,and 95 patients admitted from May 2020 to April 2021 were set as the observation group.The control group received routine nursing,while the observation group received stepwise rehabilitation training combined with music therapy on the basis of the control group.The scores of depression,anxiety,pain,and Oswestry Disability Index were compared between two groups at 1 day,14 days,1 month,3 months,and 6 months after surgery.Results At 1 month,3 months,and 6 months after surgery,the depression and anxiety scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The results of repeated measurement analysis of variance showed that,there were interaction,inter group and time effects in depression and anxiety scores between the two groups,and the differences were statistically significant(P<0.05).At 14 days,1 month,3 months,and 6 months after surgery,the pain and Oswestry Disability Index scores of the observation group were lower than those of the control group,with statistically significant differences(P<0.05).The results of repeated measurement analysis of variance showed that,there were interaction,inter group and time effects in pain and Oswestry Disability Index scores between the two groups,with statistically significant differences(P<0.05).Conclusions Stepwise rehabilitation training combined with music therapy can alleviate the negative emotions of postoperative patients with lumbar disc herniation,reduce postoperative pain,and improve the rehabilitation effect,which is worthy of clinical promotion and
