摘 要:目的 研究年龄和性别因素对梅毒血清特异性抗体(TP)水平的影响,分析检出患者分布特征。方法 采用回顾性研究方法,收集2019年1月至2021年9月苏州大学附属第一医院就诊患者实验室管理系统(LIS)检查相关数据,采用SPSS 26.0统计软件进行单因素和多因素线性回归模型分析TP与年龄、性别的相关性,以P<0.05为差异有统计学意义。结果 共检出3 438例,其中男性2 028例,女性1 410例,男女比例接近1.5∶1,年龄和TP的中位数(25%,75%四分位数)分别是60(49,69)岁和12.160(5.650,17.625)。以TP作为因变量进行单因素和多因素线性回归分析,患者年龄每增加1岁,TP水平会降低0.065和0.060,女性患者相较于男性的TP水平均会增加1.135,差异有统计学意义(P<0.001)。年龄对TP水平影响显著,在不同年龄组,女性相较于男性TP水平有增加趋势,在41~65岁年龄段,差异有统计学意义(P<0.05)。对分层后的年龄进行单因素分析发现,相比≤40岁组,41~65岁组和>65岁组患者TP分别会降低1.266和2.915,纳入性别后多重线性回归分析,分别降低1.129和2.695,且该降低趋势差异均具有统计学意义(均P<0.001)。结论 年龄和性别是患者梅毒血清特异性抗体水平分布差异的重要影响因素,尤其是大于41岁中老年人群,要提高梅毒感染人群分级诊疗管理和健康教育。Objective To study the influence of age and sex on serum specific antibody(TP)level of syphilis,and analyze the distribution characteristics of patients.Methods A retrospective study was conducted to collect the laboratory management system(LIS)examination data of patients admitted to the First Affiliated Hospital of Soochow University from January 2019 to September 2021.SPSS 26.0 statistical software was used to analyze the correlation between TP and age and sex by univariate and multivariate linear regression models,and P<0.05 was considered statistically significant.Results A total of 3438 patients were detected,including 2028 males and 1410 females(the ratio of male and female of 1.5∶1).Median age and TP were 60(49,69)years and 12.160(5.650,17.625)years,respectively.Using TP as a dependent variable,univariate and multivariate linear regression analysis showed that TP level decreased by 0.065 and 0.060 every one year increase in the age,and that the TP level in female patients increased by 1.135 compared with male patients(P<0.001).Age had the significant effect on TP levels.In different age groups,TP levels increased in females compared with males,and the difference was statistically significant in 41-65 years old(P<0.05).Compared with≤40 years old group,TP decreased by 1.266 and 2.915 in 41-65 years old group and>65 years old group by univariate linear regression analyses after age stratification,and 1.129,2.695 by multiple linear regression analyses after gender inclusion(all P<0.001).Conclusion Age and sex are important factors affecting the distribution of serum specific antibody levels of syphilis patients,especially the middle-aged and elderly population over 41 years old,so it is necessary to improve the hierarchical diagnosis and treatment management and the health education of syphilis infected population.
