胫骨后侧平台骨折创伤关节镜下手术治疗的疗效分析

摘　　要：目的 评估创伤关节镜运用于胫骨后侧平台骨折手术的临床效果.方法 选择湖南妇女儿童医院收治的86例胫骨后侧平台骨折病患作为样本对象.随机将其均分为对照组与观察组,然后进行平行对照研究,每组各取43例.对照组予以常规开放性术式治疗,观察组则在创伤关节镜条件下实施手术.对比两组手术前后膝关节功能评分、临床相关指标以及术后生活质量情况.结果 通过手术,观察组膝关节功能评分高于对照组,对比差异显著(P<0.001);观察组切口长度、手术时间、骨折愈合时间均短于对照组,手术出血量也少于对照组,两组对比差异显著,均P<0.001;且该组各项生活质量(机体功能、躯体疼痛、社会功能、情感职能、精神状况)评分均高于对照组,差异显著,均P<0.001.结论 胫骨后侧平台骨折手术中运用创伤关节镜临床效果优异,其更有利于提升患者膝关节功能,优化临床指标,提高患者术后生活质量.Objective To evaluate the clinical effect of traumatic arthroscopy on posterior tibial plateau fracture.Methods 86 patients with posterior tibial plateau fracture treated in our hospital were selected as sample objects.All patients were randomly divided into control group and observation group,and 43 cases were enrolled in each group.The control group received conventional open surgery,and the observation group received surgery under traumatic arthroscopy.The knee function scores,clinical indicators and postoperative quality of life of the two groups were compared.Results After operation,knee function score of observation group was higher than control group(P<0.001).Incision length,operation time and fracture healing time in the observation group were shorter than those in the control group,and the amount of surgical bleeding was also less than that in the control group(P<0.001).The scores of all quality of life(physical function,physical pain,social function,emotional function and mental status)in this group were higher than those in the control group(P<0.001).Conclusion The application of traumatic arthroscopy in the operation of posterior tibial plateau fracture has excellent clinical effect,which is more conducive to improving the knee function of patients,optimizing clinical indicators,and enhancing postoperative quality of life.
