"败酱消脓汤"联合介入方法治疗盆腔脓肿186例疗效分析

摘　　要：目的比较并分析抗生素+败酱消脓汤、抗生素+介入治疗、抗生素+败酱消脓汤+介入治疗三种方法治疗盆腔脓肿的临床疗效.方法回顾性分析2012年1月-2022年1月天津市南开医院收治的186例盆腔脓肿住院患者的临床资料,根据不同的治疗方法分为A组(抗生素+败酱消脓汤)、B组(抗生素+介入治疗)以及C组(抗生素+败酱消脓汤+介入治疗),并比较三组的腹痛消失时间、发热消退时间、相关实验室指标恢复正常时间、盆腔包块缩小程度等指标,并结合文献进行分析,探讨最有效的治疗方法.结果C组总有效率为94.28%,显著高于另外两组(P<0.05),复发率最低.发热消退时间、腹痛消失时间、白细胞恢复正常时间、CRP恢复时间、盆腔包块缩小程度优于A组、B组(P<0.05).治疗后C组WBC、CRP、PCT、盆腔包块大小下降最为明显(P<0.05).结论败酱消脓汤联合超声引导下穿刺置管引流术及抗生素治疗可有效抑制炎症反应,提高临床治疗效果,减少后遗症,防止复发,并且增加了患者抵抗力,最大限度地保留生育和内分泌功能.Objective To compare and analysis the clinical efficacy of antibiotics+Baijiang Xiaonong decoction,antibiotics+interventional therapy and antibiotics+Baijiang Xiaonong decoction+interventional therapy in the treatment of pelvic abscess.Methods The clinical data of 186 patients with pelvic abscess admitted to our department from January 2012 to January 2022 were retrospectively analyzed.According to different treatment methods,they were divided into group A(antibiotic+Baijiang Xiaonong decoction group),group B(antibiotic+interventional therapy group)and group C(antibiotic+Baijiang Xiaonong decoction+interventional therapy group).The abdominal pain recovery time,fever regression time,laboratory index recovery time,pelvic mass reduction degree and other indicators of the three groups were compared,and the literature was analyzed to explore the most effective treatment method.Results The total effective rate of group C was 94.28%,which was significantly higher than that of the other two groups(P<0.05),and the recurrence rate was the lowest.The fever regression time,abdominal pain regression time,white blood cell recovery time,CRP recovery time,and pelvic mass reduction were better than those in groups A and B(P<0.05).After treatment,WBC,CRP,PCT and pelvic mass size decreased most significantly(P<0.05).Conclusion Baijiang Xiaonong decoction combined with ultrasound-guided puncture catheter drainage and antibiotic treatment can more effectively inhibit inflammatory response,improve clinical treatment effect,reduce sequelae and prevent recurrence.And increase the body resistance,maximize the retention of fertility and endocrine function.
