中药治疗糖尿病便秘疗效和安全性的Meta分析

摘　　要：目的系统评价中药治疗糖尿病便秘的疗效和安全性。方法检索中国知网、万方数据知识服务平台、维普网、CBM、PubMed等数据库,搜集关于中药治疗糖尿病便秘的随机对照试验(randomized controlled trial,RCT),检索时间为建库至2022年5月。采用Review Manager5.4软件进行分析。结果共纳入22篇研究,共计1660例患者,其中中药组847例,对照组813例。结果显示,中药组的有效率优于对照组(RR=1.29,95%CI:1.17~1.42,P<0.05),中药组不良反应发生率少于对照组(RR=0.43,95%CI:0.25~0.71,P<0.05),中药组的复发率和排便时间评分与对照组比较,差异无统计学意义(RR=0.62,95%CI:0.19~1.99),P>0.05;(MD=-0.73,95%CI:-1.68~0.21,P>0.05),中药组在改善空腹血糖(fasting plasma glucose,FPG)水平、餐后2h血糖(2-hour post-load plasma glucose,2hPG)水平、排便难度、排便性状等方面优于对照组(MD=-0.62,95%CI:-1.02~-0.21,P<0.05;MD=-1.23,95%CI:-1.96~-0.50,P<0.05;MD=-0.42,95%CI:-0.62~-0.23,P<0.05;MD=-0.52,95%CI:-0.76~-0.28,P<0.05)。结论中药治疗糖尿病便秘疗效显著,具有一定的优势。Objective To systematically evaluate the efficacy and safety of traditional Chinese medicine in the treatment of diabetic constipation.Methods The databases such as PubMed,Embase,Cochrane library,CNKI,Wanfang,VIP and CBM were searched from the establishment of the database to May 2022.Screening of randomized controlled trial(RCT)for traditional Chinese medicine in the treatment of diabetic constipation.The literatures meeting the standards were analyzed by Review Manager5.4 software.Results A total of 22 studies involving 1660 participants(847 participants in the traditional Chinese Medicine group and 813 in control group)were included.The results of meta-analysis showed that the effective rate of traditional Chinese medicine group was superior to control group(RR=1.29,95%CI:1.17-1.42,P<0.05),the adverse reaction of traditional Chinese medicine group was less than control group(RR=0.43,95%CI:0.25-0.71,P<0.05),the recurrence rate and defecation time has no significant difference between traditional Chinese medicine group and control group(RR=0.62,95%CI:0.19-1.99,P>0.05;MD=-0.73,95%CI:-1.68-0.21,P>O.05),traditional Chinese medicine group were better than the control group in improving FPG,2hPG,difficult defecation and excrement character(MD=-0.62,95%CI:-1.02-0.21,P<0.05;MD=-1.23,95%CI:-1.96--0.50,P<0.05;MD=-0.42,95%CI:-0.62--0.23,P<0.05;MD=-0.52,95%CI:-0.76-0.28),P<0.05.Conclusion Traditional Chinese medicine are effective in the treatment of diabetic constipation,so as to have certain advantages.
