摘 要:目的:探讨宣肺止咳合剂对脂多糖(LPS)致急性肺损伤大鼠肺组织病理变化及水通道蛋白5(AQP5)表达和血清细胞因子的影响。方法:60只Wistar大鼠,随机分为正常对照组、LPS模型组、宣肺止咳合剂(7.5、15、30 mg/kg)组、阳性对照(地塞米松0.135 mg/kg)组,每组10只,连续灌胃给药14 d,测定肺组织湿干重(W/D)比值;HE染色观察肺组织病理改变;Western blot检测肺组织AQP5蛋白表达;ELISA检测血清IL-1β、IL-8、INF-γ、TGF-β水平。结果:宣肺止咳合剂组大鼠肺水肿程度减轻;宣肺止咳合剂、阳性组组大鼠肺组织炎性细胞减少、纤维化减少、空泡样管腔明显、肺泡壁结构较模型组完整,损伤较模型组明显减轻;模型组肺组织AQP5表达较正常组显著降低(P<0.01),宣肺止咳合剂中、高剂量组和阳性对照组较模型组肺组织AQP5表达显著上调(P<0.05或P<0.01);与正常组比较,模型组大鼠血清IL-1β、IL-8、TGF-β水平显著升高(P<0.01);宣肺止咳合剂中、高剂量组和阳性对照组血清IL-1β、TGF-β水平降低,且呈现浓度依赖性(P<0.05或P<0.01);与正常组比较,模型组INF-γ水平降低,宣肺止咳合剂低、中、高剂量组和阳性对照组较模型组均升高(P<0.05)。结论:宣肺止咳合剂对脂多糖所致大鼠急性肺损伤有一定的保护作用,其机制可能与上调AQP5蛋白表达,抑制IL-1β、IL-8炎症因子和纤维化因子TGF-β的释放,上调INF-γ水平有关。Objective:To explore the effect of Xuanfei Zhike Mixture on pulmonary pathological changes,AQP5 protein expression and serum cytokines in rats with acute lung injury induced by lipopolysaccharide(LPS).Methods:60 healthy SPF male Wistar rats were randomly divided into normal control group,LPS model group,Xuanfei Zhike Mixture(7.5,15,30 mg/kg)groups and positive control(0.135 mg/kg Dexamethasone)group,with 10 rats in each group,each group was given medicine or saline for 14 days.The wet dry weight(W/D)ratio of lung tissue was measured,HE staining was used to observe the pathological changes of lung tissue;The expression of AQP5 protein in lung tissue was detected by Western blot;serum IL-1β,IL-8,INF-γ,TGF-βlevels were determined by ELISA.Results:The inflammatory cells and fibrosis in the lung ti-ssue of rats in Xuanfei Zhike Mixture groups and positive group were decreased,The vacuole like lumen was obvious,the alveolar wall structure was more complete than that of model group,the pulmonary injury of rats in Xuanfei Zhike Mixture groups was significantly reduced compared with model group.The expression of AQP5 in lung tissue of model group was significantly lower than that of the normal group[(0.534±0.034)vs.(1±0.056)](P<0.01),and that in the medium and high dose groups of Xuanfei Zhike Mixture and the positive control groups were significantly higher than that of the model group(P<0.05 or P<0.01);Compared with the normal group,serum IL-1β,TGF-βand IL-8 levels in model group were raised,those in medium and high dose groups of Xuanfei Zhike Mixture and positive control group were lowered,serum INF-γwas lowered in model group,that was raised in Xuanfei Zhike Mixture and the positive control groups(P<0.05).Conclusion:Xuanfei Zhike Mixture has a certain protective effect on LPS induced acute lung injury in rats.Its mechanism may be related to up-regulating the expression of AQP5 protein and inhibiting the release of IL-1β,IL-8 and TGF-β,up-regulate INF-γlevel.
