超声介入胆囊造瘘术在急性胆囊炎患者中的应用效果

摘　　要：目的:探讨超声介入胆囊造瘘术在急性胆囊炎患者中的应用效果。方法:选取2020年1月—2022年12月桂林医学院第二附属医院收治的急性胆囊炎患者40例为研究对象,根据随机数字表法分为对照组(直接行胆囊切除术)与超声组(行超声介入胆囊造瘘术后择期行胆囊切除术),各20例。比较两组患者的手术情况、炎性因子水平、疼痛程度与症状控制时间。结果:治疗后,超声组术中出血量、切口感染率及住院时间均优于对照组,差异有统计学意义(P<0.05);超声组超敏C-反应蛋白、肿瘤坏死因子-α、白介素-6水平均低于对照组,差异有统计学意义(P<0.05);超声组VAS评分低于对照组,症状控制时间短于对照组(P<0.05);超声组视觉模拟评分法评分低于对照组,且症状控制时间短于对照组,差异有统计学意义(P<0.05)。结论:超声介入胆囊造瘘术在急性胆囊炎患者中的治疗效果显著,不仅可以改善胆囊切除治疗的手术效果,降低炎性因子水平,还能缓解疼痛、提升治疗效果,加快康复进程。Objective:To explore the application effect of ultrasonic interventional cholecystectomy in patients with acute cholecystitis.Methods:A Total of 40 patients with acute cholecystitis were selected at the Second Affiliated Hospital of Guilin Medical College from January 2020 to December 2022,and they were divided into control group(laparoscopic cholecystectomy)and the ultrasound group(ultrasonic interventionary cholecystectomy)according to the random number table method,with 20 cases in each group.The operation conditions,inflammatory factor levels,pain degree and symptom control time were compared between the two groups.Results:After treatment,the intraoperative blood loss,incision infection rate and hospitalization time of the ultrasound group were better than those of the control group,the difference was statistically significant(P<0.05);The levels of hypersensitive C-reactive protein,tumor necrosis factor-αand interleukin-6 in ultrasound group were lower than those in control group,and the difference was statistically significant(P<0.05);the VAS scores in ultrasound group was lower than that in control group,symptom control time was shorter than that in control group(P<0.05);The score of visual simulation score in ultrasound group was lower than that in control group,and the symptom control time was shorter than that in control group,the difference was statistically significant(P<0.05).Conclusion:Ultrasound interventional cholecystostomy has a significant therapeutic effect in patients with acute cholecystitis.It can not only improve the surgical effect of cholecystectomy,reduce the level of inflammatory factors,but also relieve pain,improve the therapeutic effect,and accelerate the recovery process.
