摘　　要：目的:探讨舒张早期纵向峰值应变率(e′SR)对左室壁节段性运动异常冠心病(CHD)患者冠状动脉病变严重程度的预测价值。方法:选取2020年1月至2022年1月收治的左室壁节段性运动异常的CHD患者183例,根据Gensini积分法分为低危组(n=84)和高危组(n=99),另选取同期体检的健康志愿者100例为对照组。比较3组左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVEDS)、收缩末期左心房内径(LAD)、左心房最大容积(LAV)、左心房容积指数(LAVI)、E/A、e′、E/e′、e′SR、E/e′SR以及收缩期左室整体纵向应变(GLS),采用ROC曲线分析e′SR预测左室壁节段性运动异常CHD患者冠状动脉病变严重程度的价值。结果:高危组LAV、LAVI高于对照组(P<0.05);高危组e′低于对照组和低危组,E/e′高于对照组和低危组;高危组e′SR、GLS低于对照组和低危组,E/e′SR高于对照组和低危组,且低危组e′SR、GLS低于对照组(P<0.05),E/e′SR高于对照组(P<0.05)。ROC曲线结果显示,e′SR预测左室壁节段性运动异常的CHD患者冠状动脉病变严重程度的AUC为0.769(95%CI 0.697~0.841)。结论:e′SR对左室壁节段性运动异常的CHD患者冠状动脉病变严重程度具有一定的预测价值。Aim:To explore the predictive value of early diastolic longitudinal peak strain rate(e′SR)on the severity of coronary artery lesion in patients with coronary heart disease(CHD)complicated with regional left ventricular wall motion abnormality.Methods:A total of 183 patients with CHD and regional left ventricular wall motion abnormality were selected between January 2020 and January 2022,and they were allocated into low-risk group(n=84)and high-risk group(n=99)according to Gensini score method.A total of 100 healthy volunteers who underwent physical examination during the same period were selected as control group.Left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVEDS),end-systolic left atrial diameter(LAD),maximum left atrial volume(LAV),left atrial volume index(LAVI),E/A,e′,E/e′,e′SR,E/e′SR and systolic left ventricular global longitudinal strain(GLS)were compared among the 3 groups.ROC curve was used to analyze the value of e′SR on predicting the severity of coronary artery lesion in patients with CHD and regional left ventricular wall motion abnormality.Results:LAV and LAVI in high-risk group were higher than those in the control group(P<0.05).e′in high-risk group was lower than that in control group and low-risk group(P<0.05),while E/e′was higher than that in control group and low-risk group(P<0.05).e′SR and GLS in high-risk group were lower than those in control group and low-risk group(P<0.05),while E/e′SR was higher than that in control group and low-risk group(P<0.05),and e′SR and GLS in low-risk group were lower while E/e′SR was higher compared with control group(P<0.05).ROC curve showed that the AUC of e′SR in predicting the severity of coronary artery lesion in patients with CHD complicated with regional left ventricular wall motion abnormality was 0.769(95%CI 0.697-0.841).Conclusion:e′SR has certain predictive value on the severity of coronary artery lesion in patients with CHD complicated with re
