摘　　要：目的 分析宁波市居民高血压合并肿瘤死亡原因的流行病学特征,为进一步开展人群防控工作提供基础资料和参考依据。方法 收集整理2018—2021年宁波市居民高血压合并肿瘤死亡报告资料,统计分析死亡率和标化死亡率,分性别、年龄别死亡率,不同文化程度死亡率及主要肿瘤构成比。结果 2018—2021年宁波市≥30岁居民高血压合并肿瘤死亡率为76.54/10万(标化率124.79/10万),男性为100.11/10万(标化率为164.98/10万),女性为54.16/10万(标化率为87.16/10万),男女性别高血压合并肿瘤死亡率以及不同文化程度死亡率差异有统计学意义(χ2=1 231.21,P<0.01);总人群、男性、女性高血压合并肿瘤年龄别死亡率均随年龄增加呈现上升态势(χ2=29 376.25,P<0.01);高血压合并肿瘤的种类主要为肺癌、胃癌、结直肠癌、肝癌和胰腺癌。结论 宁波市应重点防控≥60岁、男性和初中及以下文化程度人群的高血压合并肿瘤死亡情况;对于高血压人群,尤其注意加强对主要肿瘤如肺癌、结直肠癌、胃癌、肝癌和胰腺癌等的预防和控制。Objective To analyze the major causes of death in hypertensive patients with tumor in Ningbo.Methods The death data of patients with hypertension and tumor in Ningbo from 2018-2021 were obtained.The crude mortality,standardized mortality(SMR),age and gender related mortality,education related mortality,and major tumor composition ratio were calculated and discussed.Results From 2018 to 2021in Ningbo among residents aged over 30 years,the annual crude mortality of hypertensive patients with tumor was 76.54/100000(SMR was 124.79/100000),which was 100.11/100000(SMR was 164.98/100000)in males and was 54.16/100000(SMR was 87.16/100000)in females.There were significant differences in the mortality rate between males and females,among patients with different age and with different education(χ^(2)=1231.21,P<0.01).The overall crude mortality rate and death rates of females and males all increased with age(χ^(2)=29376.25,P<0.01),while decreased with education level(P<0.01).The most common malignant tumors in patients with hypertension were lung cancer,gastric cancer,colorectal cancer,liver cancer and pancreatic cancer.Conclusions Older patients with hypertension commodified with malignant tumors are more likely to die,especially older males and those poor-educated.It’s very important to screen common malignant tumors such as lung cancer,colorectal cancer,gastric cancer,liver cancer and pancreatic cancer in patients with hypertension in order for the early diagnosis and treatment.
