摘 要:目的:了解全科医学专业住院医师规范化培训(简称住培)学员对缓和医疗的认知和培训需求情况。方法:采用问卷调查法,于2022年5月对浙江省杭州、温州、宁波、丽水4个城市共8家住培基地的147名学员进行调查,了解其对缓和医疗的认知和培训需求等,采用秩和检验分析影响学员缓和医疗认知的主要因素。结果:全科医学专业住培学员的缓和医疗认知得分为(39.74±5.71)分。147名学员中,103名(70.0%)学员表示需要缓和医疗培训,100名(68.0%)学员认为有必要在社区推行缓和医疗服务,分别有94名(63.9%)和100名(68.0%)学员认为全科医生应该或有能力成为缓和医疗的主要提供者。是否有宗教信仰( Z=-2.56, P=0.010),在过去一年中失去亲人的经历( H=6.06, P=0.048)对其缓和医疗认知的差异具有统计学意义。 结论:全科医学专业住培学员对于缓和医疗认知不足,对缓和医疗培训的需求较大,对开展社区缓和医疗服务有信心,建议在全科医学专业住培中融入缓和医疗培训。Objective To get a preliminary understanding of trainees'knowledge and needs on palliative care in standardized residency training in general practice.Methods Using a questionnaire survey method,a total of 147 trainees from 8 bases in Hangzhou,Wenzhou,Ningbo and Lishui were investigated by questionnaire to understand their perception of palliative care and demand for palliative care education in May 2022.Descriptive analysis and rank sum test were used to analyze the influencing factors.Results The score of palliative care knowledge of general practice trainees was(39.74±5.71).Among the 147 trainees,103 trainees(70.0%)indicated the need for palliative care training courses,100 trainees(68.0%)believed that it was necessary to provide palliative care services in the community,94(63.9%)and 100(68.0%)trainees believed that general practitioners should be or have the ability to be the main providers of palliative care,respectively.Religious affiliation(Z=-2.56,P=0.010)and the experience of bereavement in the past year(H=6.06,P=0.048)had statistically significant differences in their cognition of palliative care.Conclusions The general practice trainees have insufficient awareness of palliative care,a large demand for palliative care education,and confidence in community palliative care services.It is suggested to integrate palliative care education into standardized residency training of general practice.
