摘　　要：【目的】探讨亚甲基四氢叶酸还原酶(MTHFR)基因C677T多态性与冠心病患者病情程度、合并症的相关性。【方法】选择2021年1月至2022年1月本院收治的496例冠心病患者资料,根据MTHFR基因C677T多态性检测结果将其分为CC组(n=139)、CT组(n=211)、TT组(n=146)。比较三组患者冠心病病情程度、血浆同型半胱氨酸(Hcy)、血压水平,血清总胆固醇(TC)、甘油三酯(TG)、糖化血红蛋白(HbA1c)水平,统计三组中H型高血压、高血压、糖尿病、高脂血症发生率;分析MTHFR基因C677T多态性分型与Gensini评分、Hcy、TC、TG、HbA1c的相关性。【结果】三组患者Hcy、SBP、DBP、TC、TG、HbA1c、Gensini评分比较,差异均具有统计学意义(P<0.05)。三组患者H型高血压、高血压、糖尿病、高脂血症合并症发生率比较,差异均具有统计学意义(P<0.05)。【结论】MTHFR基因C677T多态性与冠心病患者病情程度、Hcy、血脂及血糖水平具有相关性,其中TT基因型冠心病患者病情程度严重的概率较高,且合并H型高血压、高血压、糖尿病、高脂血症的风险也更高。【Objective】To investigate the correlation between the C677T polymorphism of the methylenetetrahydrofolate reductase(MTHFR)gene and the severity and comorbidities of coronary heart disease patients.【Methods】A total of 496 patients with coronary heart disease admitted to our hospital from January 2021 to January 2022 were selected and divided into CC group(n=139),CT group(n=211),and TT group(n=146)based on the MTHFR gene C677T polymorphism detection results.The severity of coronary heart disease,plasma homocysteine(Hcy),blood pressure,and serum total cholesterol(TC),triglyceride(TG),and glycosylated hemoglobin(HbA1c)were compared among the three groups,and the incidence of H-type hypertension,hypertension,diabetes,and hyperlipidemia in the three groups were statistically analyzed;We analyzed the correlation between MTHFR gene C677T polymorphism typing and Gensini score,Hcy,TC,TG,HbA1c.【Results】The differences in Hcy,SBP,DBP,TC,TG,HbA1c,and Gensini scores among the three groups of patients were statistically significant(P<0.05).The incidence rate of H-type hypertension,hypertension,diabetes and hyperlipidemia complications in the three groups were statistically significant(P<0.05).【Conclusion】The C677T polymorphism of MTHFR gene is associated with the severity,Hcy,blood lipid and blood glucose levels of patients with coronary heart disease.Among them,patients with TT genotype coronary heart disease have a higher probability of severity,and have a higher risk of combining H-type hypertension,hypertension,diabetes and hyperlipidemia.
