低频脉冲电流穴位刺激联合常规针灸及康复训练治疗BrunnstromⅣ期脑卒中后偏瘫41例临床观察

摘　　要：目的观察低频脉冲电流穴位刺激联合常规针灸及康复训练治疗BrunnstromⅣ期脑卒中后偏瘫的临床疗效及对患者肢体功能与日常生活活动能力的影响。方法对93例BrunnstromⅣ期脑卒中后偏瘫患者的临床资料进行回顾性分析,根据治疗方案不同分为对照组52例、治疗组41例。对照组予康复训练及常规针灸治疗,治疗组在对照组治疗方法的基础上联合低频脉冲电流穴位刺激。治疗3个月后比较2组的临床疗效以及治疗前后的运动功能评分和改良Barthel指数(MBI)评分。结果对照组总有效率为76.93%(40/52),治疗组为92.68%(38/41),2组比较差异有统计学意义(P<0.01);治疗后2组运动功能评分及MBI评分明显升高,与同组治疗前比较差异均有统计学意义(P<0.01),且治疗组升高更明显,与对照组治疗后比较差异均有统计学意义(P<0.01)。结论低频脉冲电流穴位刺激联合常规针灸及康复训练治疗BrunnstromⅣ期脑卒中后偏瘫临床疗效显著,可明显改善患者的肢体功能及日常生活活动能力,值得临床推广应用。Objective To observe the clinical effect of low-frequency pulse current acupoint stimulation combined with conventional acupuncture and rehabilitation training in the treatment of post-stroke hemiplegia in Brunnstrom stageⅣand the effect on the limb function and activities of daily living of patients.Methods A retrospective analysis was conducted on the clinical data of 93 patients with post-stroke hemiplegia in Brunnstrom stageⅣ.According to different treatment plans,they were divided into a control group of 52 cases and a treatment group of 41 cases.The control group was given rehabilitation training and conventional acupuncture and moxibustion treatment.The treatment group was combined with low-frequency pulse current acupoint stimulation on the basis of the treatment method of the control group.After 3 months of treatment,the clinical efficacy,motor function scores,and modified Barthel index(MBI)scores of the two groups were compared before and after treatment.Results The total effective rate of the control group was 76.93%(40/52),while that of the treatment group was 92.68%(38/41).The difference between the two groups was statistically significant(P<0.01).After treatment,the motor function scores and MBI scores of the two groups significantly increased,with statistical significance(P<0.01)compared to the same group before treatment,and the increase was more significant in the treatment group,with statistical significance(P<0.01)compared to the control group after treatment.Conclusion Low-frequency pulse current acupoint stimulation combined with conventional acupuncture and rehabilitation training has a significant clinical effect on post-stroke hemiplegia in Brunnstrom stageⅣ,which can significantly improve the limb function and activities of daily living of patients,and is worthy of clinical promotion and application.
