新加黄龙汤加减联合掀针治疗混合痔术后便秘阴虚肠燥证30例临床观察

摘　　要：目的观察新加黄龙汤加减联合掀针治疗混合痔术后便秘阴虚肠燥证的临床疗效。方法将60例混合痔术后便秘阴虚肠燥证患者采用随机数字表法分为对照组和治疗组,各30例。对照组给予乳果糖口服液治疗,治疗组给予新加黄龙汤加减口服配合掀针治疗。治疗10 d后观察2组的临床疗效,以及治疗前后便秘症状评分、视觉模拟评分法(VAS)评分及血清P物质(SP)、血管活性肽(VIP)水平。结果治疗组总有效率为93.33%(28/30),对照组为70.00%(21/30),2组比较差异有统计学意义(P<0.05);治疗后2组便秘症状评分、血清VIP水平明显降低,血清SP水平明显升高,与同组治疗前比较差异均有统计学意义(P<0.05或P<0.01),且治疗组改善更明显,与对照组治疗后比较差异有统计学意义(P<0.05);治疗后第5、第10天2组VAS评分均明显降低,与同组治疗前比较差异有统计学意义(P<0.05或P<0.01),且治疗组降低更明显,与对照组同期比较差异均有统计学意义(P<0.05)。结论新加黄龙汤加减联合掀针治疗混合痔术后便秘阴虚肠燥证临床疗效显著,可有效促进排便,改善便秘症状,同时缓解患者术后疼痛,其可能通过调节血清SP和VIP水平而发挥作用。Objective To observe the clinical efficacy of modified Xinjia Huanglong Tang(新加黄龙汤)combined with needle-embedding therapy in the treatment of constipation with yin deficiency and intestinal dryness after mixed hemorrhoid operation.Methods 60 patients with constipation yin deficiency and intestinal dryness after mixed hemorrhoids operation were divided into control group and treatment group by random number table method,with 30 cases in each group.The control group was treated with lactulose oral liquid,and the treatment group was treated with modified Xinjia Huanglong Tang combined with acupuncture.After 10 days of treatment,the clinical efficacy,c onstipation symptom score,visual analogue scale(VAS)score,serum Substance P(SP)and vasoactive peptide(VIP)levels of the 2 groups were observed.Results The total effective rate of the treatment group was 93.33%(28/30),while that of the control group was 70.00%(21/30).The difference between the two groups was statistically significant(P<0.05).After treatment,the constipation symptom score and serum VIP level in both groups were significantly decreased,while the serum SP level was significantly increased.The difference was statistically significant compared to the same group before treatment(P<0.05 or P<0.01),and the improvement was more significant in the treatment group.The difference was statistically significant compared to the control group after treatment(P<0.05).On the 5th and 10th day after treatment,the VAS scores of both groups significantly decreased,with a statistically significant difference compared to the same group before treatment(P<0.05 or P<0.01),and the decrease was more significant in the treatment group,with a statistically significant difference compared to the control group at the same period(P<0.05).Conclusion The combination of modified Xinjia Huanglong Tang and needle-embedding therapy has a significant clinical effect in the treatment of constipation with yin deficiency and intestinal dryness after mixed hemorrhoids surgery.It can eff
