情志护理联合耳穴贴压改善重症患者谵妄的临床研究

摘　　要：目的探讨情志护理联合耳穴贴压改善重症患者谵妄的临床效果。方法选择80例重症并发谵妄患者,以随机数字法分为2组,每组40例,对照组采用常规护理,观察组采用情志护理联合耳穴贴压,对比两组患者的NEECHAM评分、PSQI评分,谵妄持续时间,血清皮质醇(Cor)、血糖(Glu)、肾上腺素(E)、去甲肾上腺素(NE)水平以及心理状态。结果护理前两组各项数据无差异。护理后观察组NEECHAM评分[(15.45±2.11)分]高于对照组[(12.38±2.21)分],PSQI评分[(7.27±1.25)分]低于对照组[(10.52±2.21)分],P<0.05;观察组患者谵妄持续时间[(9.32±2.05)天]短于对照组[(14.16±3.22)天],P<0.05;观察组血清Cor[(259.12±5.43)μg/L]、Glu[(5.66±0.54)mmol/L]、E[(52.01±8.21)μg/L]、NE[(312.04±8.34)μg/L]水平低于对照组[(269.01±5.54)μg/L]、(6.78±0.61)mmol/L、(59.44±8.33)μg/L、(376.55±8.55)μg/L,P<0.05。护理后观察组T-AI评分[(27.76±1.65)分]、S-AI评分[(30.43±1.32)分]低于对照组[(41.54±2.11)分、(45.11±2.03)分],P<0.05。结论情志护理联合耳穴贴压可有效改善患者的意识和睡眠情况,从而缩短其谵妄持续时间,促进精神状态改善,显著改善患者的临床指标,适合在临床实施和开展。Objective To explore the clinical effect of emotional nursing combined with auricular point sticking on severe patients with delirium.Methods 80 patients with severe delirium were randomly divided into two groups:control group(n=40)and observation group(n=40),NEECHAM score,PSQI score,delirium duration,serum Cor,Glu,E,NE levels and psychological status were compared between the two groups.Results After nursing,NEECHAM score(15.45±2.11)in observation group was higher than that in control group(12.38±2.21),and PSQI score(7.27±1.25)was lower than that in control group(10.52±2.21),P<0.05.The duration of delirium in the observation group[(9.32±2.05)days]was shorter than that in the control group[(14.16±3.22)days],P<0.05.The levels of COR[(259.12±5.43)μg/L],Glu[(5.66±0.54)mmol/L],E[(52.01±8.21)μg/L],NE[(312.04±8.34)μg/L]in the observation group were lower than those in the control group[(269.01±5.54)μg/L,(6.78±0.61)μg/L,(59.44±8.33)μg/L,(376.55±8.55)μg/L],P<0.05.The T-AI score(27.76±1.65)and S-AI score(30.43±1.32)in the observation group were lower than those in the control group[(41.54±2.11)and(45.11±2.03)],P<0.05.Conclusion Emotional nursing combined with auricular-plaster therapy can effectively improve the consciousness and sleep of patients,thus shorten the duration of delirium,promote the improvement of mental state,and significantly improve the clinical indexes of patients,suitable for clinical implementation and development.
