血清脂蛋白相关磷脂酶A2水平与冠状动脉病变程度及慢血流的相关性分析

摘　　要：目的探讨血清脂蛋白相关磷脂酶A2(LP-PLA2)水平与冠状动脉病变程度及慢血流的相关性。方法选取2020年12月至2021年8月收治的80例冠心病患者作为研究对象,对所有患者进行血清LP-PLA2检测,根据LP-PLA2水平进行分组,LP-PLA2水平正常(<175 ng/mL)的44例患者为对照组,LP-PLA2水平异常(≥175 ng/mL)的36例患者为观察组。将观察组患者按LP-PLA2水平分为低水平组(175~207 ng/mL,16例)和高水平组(>207 ng/mL,20例)。比较各组的Gensini积分、校正的TIMI帧数(CTFC)及空腹血糖(FBG)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、同型半胱氨酸(HCY)、纤维蛋白原(FIB)、脂蛋白相关磷脂酶A2(LP-PLA2)水平,分析LP-PLA2与各项指标的相关性。结果观察组的Gensini积分、CTFC及FBG、TG、LDL-C、HCY、LP-PLA2水平均高于对照组,差异具有统计学意义(P<0.05)。低水平组的Gensini积分、CTFC及FBG、TG、LDL-C、HCY、LP-PLA2水平均低于高水平组,差异具有统计学意义(P<0.05)。Pearson相关性分析结果显示,LP-PLA2水平与Gensini积分、CTFC及FBG、TG、LDL-C、HCY水平显著相关(P<0.05)。结论血清LP-PLA2水平能有效反映冠状动脉病变程度,也能一定程度上预测慢血流的发生。LP-PLA2与冠状动脉病变程度和慢血流具有显著相关性,值得临床推广与应用。Objective To investigate the correlation between serum lipoprotein-associated phospholipase A2(LP-PLA2)level and severity of coronary artery disease and slow blood flow.Methods A total of 80 patients with coronary heart disease admitted from December 2020 to August 2021 were selected as the research objects.Serum LP-PLA2 was detected in all patients.According to the level of LP-PLA2,the patients were grouped.Forty-four patients with normal LP-PLA2 level(<175 ng/mL)were included in control group,and 36 patients with abnormal LP-PLA2 level(≥175 ng/mL)were included in observation group.According to the level of LP-PLA2,the patients in the observation group were divided into low level group(175-207 ng/mL,16 cases)and high level group(>207 ng/mL,20 cases).The Gensini score,corrected TIMI frame count(CTFC)and fasting blood glucose(FBG),triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),homocysteine(HCY),fibrinogen(FIB)and lipoprotein-associated phospholipase A2(LP-PLA2)levels were compared among the groups,and the correlation between LP-PLA2 and each index was analyzed.Results The Gensini score,CTFC and FBG,TG,LDL-C,HCY and LP-PLA2 levels in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The Gensini score,CTFC and FBG,TG,LDL-C,HCY and LP-PLA2 levels in the low level group were lower than those in the high level group,and the differences were statistically significant(P<0.05).The pearson correlation analysis results showed that LP-PLA2 level was significantly correlated with Gensini score,CTFC and FBG,TG,LDL-C and HCY levels(P<0.05).Conclusion Serum LP-PLA2 level can effectively reflect the severity of coronary artery disease,and can also predict the occurrence of slow flow to a certain extent.LP-PLA2 has a significant correlation with the degree of coronary artery disease and slow blood flow,which is worthy of promotion and application in clinic.
