摘  要:目的探讨3D打印技术辅助个性化设计耳后支架在耳郭再造术中的应用效果。方法自2021年6月至2022年8月,郑州大学第一附属医院医学美容中心将3D打印技术应用于39例先天性小耳畸形患者二期颅耳角重建手术。术前利用三维扫描技术获取健侧耳郭、耳甲腔侧壁相关数据,据此设计、打印耳后支架3D模型,术中根据该模型雕刻肋软骨制备对应耳后支架,完成个性化颅耳角重建。术后6个月,对比分析、评估两侧耳郭颅耳角的角度及形态,并评价满意度。结果39例患者术后获随访6~20个月,其中37例颅耳角角度及形态与健侧高度一致,对称性良好,满意度较高;2例因术后瘢痕挛缩致颅耳角角度略小于健侧,但整体效果基本满意。结论在耳郭再造术中,针对不同患者,3D打印技术可辅助实现耳后支架的个性化设计、雕刻与应用,进而提升再造耳颅耳角及形态的对称度、提高手术的满意度,并避免多次修复给患者带来的心理负担和经济压力。Objective To explore the application and effectiveness of 3D printing technology assisted personalized design of ear support in auricular reconstruction surgery.Methods From June 2021 to August 2022,3D printing technique was applied to patients with congenital microtia who were scheduled for the auriculocephalic angle reconstruction.Preoperatively,the relevant data of the concha cavity of the contralateral auricular were obtained by three-dimensional scanning technology,and the model of the retroauricular framework was designed and printed.Intraoperatively,the costal cartilage carved according to the model was used to the reconstruction of auriculocephalic angle and morphology.After 6 months of surgery,comparative analysis and evaluation of the angle and shape of the cranial ear angles on both sides of the auricles were conducted,and satisfaction was evaluated.Results Thirty-nine patients were followed up for 6 to20 months.In 37 patients,the reconstructed auriculocephalic angle and morphology were symmetrical with the normal side,and the results were satisfactory.In 2 patients,the reconstructed auriculocephalic angle became smaller due to scar contracture,and the results was basically satisfactory.Conclusion In auricle reconstruction,3D printing technology can realize the design,carving and application of personalized retroauricular framework,so as to improve the symmetry of reconstructed auriculocephalic angle and morphology,improve the satisfaction of operation and avoid the psychological and economic pressure caused by repeated repair operation.
