摘　　要：目的探讨采用游离足第二趾胫侧皮瓣修复手指甲床缺损伤的临床效果。方法自2010年6月至2018年7月,吴江瑞兴医院骨科采用游离足第二趾胫侧皮瓣修复16例手指甲床缺损伤患者,皮瓣面积为1.8 cm×1.6 cm~2.4 cm×2.6 cm,足部供区采用足背或小腿全厚皮片植皮。结果16例患者术后皮瓣全部一期成活,足部供区植皮均成活。所有患者术后获随访6~24个月,平均14个月。游离皮瓣色泽与邻近皮肤相近,修复后指功能良好,外形满意,足部行走、穿鞋无明显异常。根据中华医学会手外科学会上肢功能评定试用标准评定:优12例,良3例,可1例,优良率93.8%。结论采用游离足第二趾胫侧皮瓣修复手指甲床缺损伤,具有操作简便,供区损伤较小,手指外形、功能恢复满意的优点,是一种比较理想的方法。Objective To explore the clinical effects of free secondtoe medial flaps for repair of digital nail bed defects.Methods From June 2010 to July 2018,16 cases with digital nail bed defects were treated by free second toe medial flaps in our department.The size of the flap was from 1.8 cm×1.6 cm to 2.4 cm×2.6 cm.The wound of the donor sites was repaired by the full-thickness skin graft from the dorsum of the foot or lower leg.Results All 16 flaps survived in one stage.The skin graft of the foot donor site survived in all the cases.Postoperatively the patients were follow-up for 6 to 24 months with an average of 14 months.The contour of the free flap was satisfactory and its color was similar to that of the adjacent skin.The appearance of the repaired finger was satisfactory.All the patients walked and wore shoes well without obvious abnormality.According to the trial criteria for evaluation of upper limb function of the Society of Hand Surgery of the Chinese Medical Association,the excellent was in 12 cases,the good was in 3 cases and the fair was in 1 case.The excellent and good rate was 93.8%.Conclusion It is an ideal method to repair digital nail bed defects using free second-toe medial flaps.This method is simple with tiny damage in the donor site.The appearance and functional recovery of the repaired finger are satisfactory.
