茵泽温肝方治疗寒湿困脾型胆汁淤积性性肝病疗效及对患者肝功能的影响

摘　　要：目的:观察茵泽温肝方治疗寒湿困脾型胆汁淤积性肝病疗效及对患者肝功能的影响。方法:将152例寒湿困脾型胆汁淤积性肝病患者依据随机数字表法分为对照组和试验组各76例,对照组在常规保肝治疗基础上给予熊去氧胆酸、腺苷蛋氨酸治疗,试验组在对照组基础上口服茵泽温肝汤,治疗3个月后比较两组患者临床疗效、血清总胆红素(total bilirubin,TBIL)、碱性磷酸酶(alkaline phosphatase,ALP)、肝纤维指标水平及肝脏硬度值终末期肝病模型评分(liver hardness value end-stage liver disease model score,MELD)、Child-pugh分级变化情况。结果:对照组与试验组总有效率分别为50.00%(38/76)、75.00%(57/76)(P<0.05);与治疗前相比,治疗后两组患者TBIL、ALP水平与肝纤维指标均下降,试验组下降更明显(P<0.05);与治疗前相比,治疗后对照组MELD评分变化不明显(P>0.05),试验组MELD评分降低(P<0.05);与治疗前相比,两组Child-pugh分级均改善,试验组改善程度优于对照组(P<0.05)。结论:茵泽温肝方能减轻寒湿困脾型胆汁淤积性肝病患者肝纤维化程度,改善肝脏硬度值,并能提高临床疗效。Objective:To observe curative effects of Yinze Wengan prescription in the treatment of cholestatic liver disease of cold-damp encumbering spleen pattern and its influence on patients'liver function.Methods:All 152 patients were divided into control group and experiment group according to random number table method(n=76 cases),the control group was treated with ursodesoxycholic acid and ademetionine based on conventional liver-protecting therapy,and the experiment group took Yinze Wengan prescription orally on the foundation of the therapy the control group accepted,after three months of the treatment,to compare clinical effects,the levels of TBIL,ALP and the indexes of liver fibrosis,MELD and Child-pugh grading changes between both groups.Results:Total effective rates of the control group and the experiment group were 50.00%(38/76)and 75.00%(57/76)respectively(P<0.05);compared with before the treatment,the levels of TBIL and ALP,and the indexes of liver fibrosis in both groups lowered,and the decrease of the experiment group was more noticeable(P<0.05);compared with before the treatment,the changes of MELD scores were insignificant in the control group after the treatment(P>0.05),MELD scores decreased in the experiment group(P<0.05);compared with before the treatment,Child-pugh grading of the two group improved,the improvements of Child-pugh grading of the experiment group were better than these of the control group(P<0.05).Conclusion:Yinze Wengan prescription could relieve liver fibrosis degrees in patients with cholestatic liver disease of cold-damp encumbering spleen pattern,improve liver hardness value and raise clinical effects.
