摘 要:目的:探讨PCR-荧光探针法在老年下呼吸道感染患者病原体检测中的应用价值,为临床提供早期诊断依据。方法:收集2022年3月至2023年2月在曲靖市第五人民医院门诊及住院部治疗的300例老年下呼吸道感染患者的口咽或鼻咽拭子标本,采用PCR-荧光探针法对标本中的甲型流感病毒(IFVA)、乙型流感病毒(IFVB)、呼吸道合胞病毒(RSV)、腺病毒(ADV)、副流感病毒1型(HPIV1)、副流感病毒3型(HPIV3)6种呼吸道病毒进行检测,分析检测结果。结果:入选的300份标本中,检出阳性51份,检出率为17.00%。51份阳性标本中,阳性检出率最高的是流感病毒(IFVA+IFVB),其次为ADV,HPIV3检出率最低。不同性别患者呼吸道病毒的阳性检出率相比无统计学差异(P>0.05)。不同年龄(60~69岁、70~79岁、≥80岁)人群呼吸道病毒的阳性检出率相比无统计学差异(P>0.05)。春季呼吸道病毒的阳性检出率显著高于其他三个季节,差异有统计学意义(P<0.05)。结论:临床采用PCR-荧光探针法对老年下呼吸道感染患者进行病原体检测便捷快速,具有较高的临床应用价值。Objective:To investigate the application value of PCR-fluorescence probe in pathogen detection in elderly patients with lower respiratory tract infection,and to provide early diagnosis basis for clinical practice.How:Oral and nasopharyngeal swab samples were collected from 300 elderly patients with lower respiratory tract infection treated in the outpatient and inpatient department of the Fifth People’s Hospital of Qujing City from March 2022 to February 2023.Pcr-fluorescent probe method was used to detect influenza A virus(IFVA),influenza B virus(IFVB),respiratory syncytial virus(RSV),adenovirus(ADV),parainfluenza virus type 1(HPIV1)and Parainfluenza virus type 3(HPIV3)in the samples.Results:Among the 300 samples,51 were positive,the detection rate was 17.00%.Among the 51 positive samples,influenza virus(IFVA+IFVB)had the highest positive rate,followed by ADV,and HPIV3 had the lowest positive rate.There was no significant difference in the positive rate of respiratory virus among different genders(P>0.05).There was no significant difference in the positive rate of respiratory virus among different age groups(60-69 years old,70-79 years old,≥80 years old)(P>0.05).The positive detection rate of respiratory virus in spring was significantly higher than that in other three seasons,and the difference was statistically significant(P<0.05).Conclusion:PCR-fluorescence probe is a convenient and rapid method for pathogen detection in elderly patients with lower respiratory t ract infection,and has high clinical application value.
