环境重金属钼和锡暴露与糖尿病患病风险的交互作用

摘  要:目的探讨重金属钼和锡暴露与糖尿病患病风险的相关性及潜在的交互作用,以期为环境重金属暴露对糖尿病等代谢性疾病的作用提供理论依据。方法基于美国国家健康和营养调查数据平台,采用横断面研究方法,将确诊糖尿病患者作为病例组,运用非条件Logistic回归模型评估尿液钼质量分数和锡质量分数与糖尿病患病风险的相关关系。结果共纳入5294例成年人,其中995例患糖尿病。当调整混杂因素后,与肌酐矫正尿液钼质量分数和锡质量分数高于其均值(37.96μg/g和0.50μg/g)的人群相比,糖尿病患病风险分别增加76%和66%。以肌酐矫正的尿液钼质量分数和锡质量分数均低于均值的人群作为对照组,2种重金属质量分数均高于均值的人群糖尿病患病风险增加170%。结论尿液重金属钼和锡暴露增加糖尿病的患病风险,当2种重金属质量分数均高于均值时,糖尿病患病风险最大。Objective To explore the association between exposure to heavy metals including molybdenum and tin and the risk of diabetes,as well as their potential interactive effects.Methods Based on the U.S.National Health and Nutrition Examination Survey,a cross-sectional study design was used.Participants diagnosed with diabetes were considered the case group.Non-conditional Logistic regression models were employed to evaluate the correlation between urinary molybdenum and tin concentrations and the risk of diabetes.Results A total of 5294 adults were included,of which 995 individuals had diabetes.After adjusting for confounding factors,individuals with creatinine corrected urinary molybdenum and tin concentrations higher than the mean values(37.96μg/g and 0.50μg/g)had a 76%and 66%increased risk of diabetes,respectively,compared with those with concentrations below the mean values.When comparing individuals with both creatinine corrected metal concentrations above the mean values to those the mean values,the risk of diabetes increased by 170%.Conclusion Urinary exposure to heavy metals like molybdenum and tin increases the risk of diabetes,and the highest risk occurs when both metal concentrations are above the mean values.
