基于石学敏“调神”思想浅析针刺防治围手术期神经认知功能障碍的理论依据及临床应用

摘　　要：在石学敏院士“调神”思想指导下,针刺防治围手术期神经认知功能障碍(PND)的临床研究逐渐开展。PND的关键病机为“神机不运”,在针刺治疗上把握“调神”这一基本原则,注重对“神机”的干预,通过运神使神机健利,养神使神明充沛,达到调神醒脑,恢复神机的目的。其潜在效应机制可能与针刺调控神经递质释放和促进脑功能恢复有关。本文旨在探析针刺治疗PND的相关理论机制及应用,以期为临床治疗PND提供新的防治思路和理论支持。Under the guidance of academician Shi Xuemin's“regulating spirit”ideology,clinical research of acupuncture on the prevention and treatment of perioperative neurocognitive dysfunction has been gradually carried out.The key pathogenesis of perioperative neurocognitive dysfunction is“malfunctioning of spiritual mechanism”,the location of the disease is in the brain,and the official orifices are the pathways.Smooth movement of Qi and blood is the functioning basis,and,therefore,the basic principle of“regulating spirit”should be grasped for acupuncture treatment,focusing on the intervention of“the spiritual mechanism”.Spirit mechanism healthy is usually achieved through the transportation of spirit Qi,and bright spirit abundance is usually obtained by nourishing the spirit to achieve the purpose of regulating spirit and opening the orifice and restoring the spiritual mechanism.The potential effect mechanism may be related to the regulation action of acupuncture on neurotransmitter release and promotion of brain function recovery.The purpose of this paper is to explore the related theoretical mechanism and application of acupuncture in the treatment of PND,with a view to providing new prevention and treatment ideas and theoretical support for the clinical treatment of PND.
