摘　　要：IgA肾病(IgAN)在我国是一种常见的原发性肾小球疾病,易进展至终末期肾病(ESRD)。本文通过查阅与IgAN国内外文献,从西医角度总结该病发病机制,回顾现有病理诊断评定方法及特异性生物标志物,归纳支持治疗、激素及免疫抑制剂治疗方法,并对中医医家对于该病病因病机、分期论治、辨证分型及专方专药的认识进行归类,以期对IgAN的诊治提供有效的参考意见。IgA nephropathy(IgA N)is a common primary glomerular disease in China,which is prone to progression to end-stage renal disease(ESRD).By reviewing the domestic and international literature on IgA nephropathy and from the perspective of Western medicine,this paper summarized the pathogenesis of IgA nephropathy.Specifically,we sorted and summarized the pathogenesis from the perspectives of the“multiple hits”theory,molecular structure,abnormal activation of the complement system,mucosal immune abnormalities and inflammation,environmental and genetic comprehensive factors,etc.We reviewed the existing pathological diagnosis and assessment methods and specific biomarkers,summarized supportive therapy,and hormone and immunosuppressive therapies.Based on TCM thought,IgA nephropathy was categorized according to the etiology and pathogenesis of the disease,treatment based on staging,syndrome identification and classification,and specific formula and medicinals for a specific condition.The purpose of this is to provide effective reference for the diagnosis and treatment of IgA nephropathy.
