摘  要:目的 探讨益肺宣肺降浊方通过胆碱能系统抗血管痴呆的作用及机制。方法 建立中动脉栓塞(middle cerebral artery occlusion, MCAO)大鼠血管性痴呆模型;给予益肺宣肺降浊方及联合应用胆碱受体拮抗剂山莨菪碱及其激动剂毛果芸香碱。具体分组:假手术组,模型组,益肺宣肺降浊方治疗组,益肺宣肺降浊方联合山莨菪碱组,益肺宣肺降浊方联合毛果芸香碱组,石杉碱组。水迷宫实验检测动物神经行为学;苏木精-伊红(HE)染色检测海马组织病理;透射电镜检测海马组织细胞超微结构;免疫荧光检测海马组织神经元MAP2及突触蛋白Synapsin表达。结果 益肺宣肺降浊方能有效提高血管性痴呆动物的学习和记忆能力,对海马组织病理改变具有显著的缓解作用,对神经元树突生长异常以及海马组织细胞超微结构的改变具有明显的改善作用。山莨菪碱有拮抗而毛果芸香碱有协同益肺宣肺降浊方对血管性痴呆动物的作用。结论 益肺宣肺降浊方可能通过调控胆碱能系统缓解血管性痴呆动物海马组织的病理改变、神经元细胞的超微结构改变、神经元突触生长异常从而改善血管性痴呆的认知功能障碍。Objective To investigate the effect and mechanism of Yifei Xuanfei Jiangzhuo Formula(益肺宣肺降浊方)on vascular dementia through cholinergic system.Methods The vascular dementia model of middle cerebral artery occlusion(MCAO)rats was established.Yifei Xuanfei Jiangzhuo Formula was given and combined with the choline receptor antagonist anisodamine and its agonist rutin.Specific groups were:sham operation group,model group,Yifei Xuanfei Jiangzhuo Formula treatment group,Yifei Xuanfei Jiangzhuo Formula combined with anisodamine group,Yifei Xuanfei Jiangzhuo Formula combined with pilocarpine group and huperzine group.Water maze test was used to detect the animal neurobehavior.Hippocampal histopathology was detected by HE staining.Ultrastructure of hippocampal cells was detected by transmission electron microscopy.Immunofluorescence was used to detect the expressions of MAP2 and Synapsin in hippocampal neurons.Results The results showed that Yifei Xuanfei Zhizhuo Decoction could effectively improve the learning and memory ability of animals with vascular delusion,significantly alleviate the pathological changes of hippocampal tissue and significantly improve the abnormal growth of neuron dendrite and the ultrastructure of hippocampal tissue.There were antagonistic effect of anisodamine and synergistic effect of pilocarpine combined with Yifei Xuanfei Zhizhuo Decoction on vascular dementia.Conclusion Yifei Xuanfei Zhizhuo Decoction may improve the cognitive dysfunction of vascular dementia by regulating cholinergic system and alleviating the pathological changes of hippocampal tissue,ultrastructural changes of neuron cells and abnormal synaptic growth.
