中频脉冲联合三金汤对输尿管结石碎石术后辅助排石及预后的影响分析

摘　　要：目的 探讨中频脉冲联合三金汤对输尿管结石碎石术后辅助排石及预后的影响。方法 100例输尿管结石患者按照随机数表法分为观察组和对照组各50例,对照组采用内镜碎石术并于术后采用中频脉冲治疗,观察组在对照组基础上联合服用三金汤,观察两组手术前后疼痛评分情况、术后7 d和14 d排石率、炎性指标以及并发症等。结果 术后7 d、14 d观察组结石排净率分别为64.00%(32/50)、94.00%(47/50),显著高于对照组的36.00%(18/50)、76.00%(38/50),差异具有统计学意义(P<0.05);术前两组视觉模拟评分法(Visual analogue scale, VAS疼痛评分)、C反应蛋白(C-reactive protein,CRP)、血清降钙素原(Procalcitonin, PCT)等炎性指标均无显著差异,术后观察组VAS疼痛评分、CRP、PCT水平分别为(2.14±1.16)、(11.09±1.87) mg/L、(0.47±0.13) ng/L,显著低于对照组的(3.75±1.57)、(15.37±2.18) mg/L、(0.59±0.12) ng/L,差异具有统计学意义(P<0.05);且术后观察组不良反应发生率(4.00%,2/50)显著低于对照组(12.00%,6/50),差异具有统计学意义(P<0.05)。结论 中频脉冲联合三金汤可以有效提高患者结石排净率,缩短排石时间,同时降低患者术后疼痛以及术后炎性指标水平,降低感染、出血等并发症的发生,值得临床推广。Objective To investigate the effect of intermediate frequency pulse combined with Sanjin Decoction(三金汤)on assisted expulsion and prognosis of ureteral calculi after lithotripsy.Methods A total of 100 patients with ureteral calculi were selected as the research objects,and divided into the observation group and the control group according to the random number table method,with 50 patients in each group.The patients in the control group were treated with endoscopy.Lithotripsy was treated with intermediate frequency pulse after operation.The patients in the observation group were given Sanjin Decoction on the basis of the control group.The pain scores before and after the operation,the rate of stone expulsion on the 7th and 14th day after the operation,the inflammatory indexes and complications were observed in the two groups.Results On the 7th and 14th day after the operation,the stone removal rates of the observation group were 64.00%(32/50)and 94.00%(47/50),respectively,which were significantly higher than those of the control group[36.00%(18/50)and 76.00%(38/50)],and the difference was statistically significant(P<0.05).There was no significant difference in the visual analogue scale(VAS)score,C-reactive protein(CRP),procalcitonin(PCT)or other inflammatory indicators between the two groups.The postoperative VAS score in the observation group was(2.14±1.16)points,the levels of CRP and PCT were(11.09±1.87)mg/L and(0.47±0.13)ng/L,respectively,significantly lower than those[(3.75±1.57)points,(15.37±2.18)mg/L,(0.59±0.12)ng/L]in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group after surgery(4.00%,2/50)was significantly lower than that(12.00%,6/50)of the control group and the difference was statistically significant(P<0.05).Conclusion Intermediate frequency pulse combined with Sanjin Decoction can effectively improve the stone removal rate,shorten the stone removal time,reduce the postoperative pain and postoperative inflam
