强直性脊柱炎证候规范化及客观化的研究与思考

摘　　要：强直性脊柱炎(Ankylosing Spondylitis, AS)是一种起病隐匿的、慢性炎症性自身免疫病,在中医学中属于“大偻”范畴,随着病情进展可伴有不同程度的皮肤黏膜、眼、肺、胃肠道、肾等多系统损害。由于生活环境及生活方式的不断变化,近年来AS的发病率呈现上升的趋势,而中医辨证论治AS在缓解炎症、减轻疼痛、改善关节功能等方面有明显优势,是中医治疗的优势病种之一。AS中医证候规范化、客观化研究对于规范临床辨证论治、提高临床疗效、推动临床科研工作的开展具有重要意义。就强直性脊柱炎证候规范化及客观化研究进展作一综述,主要综述近年来AS辨证分型的规范化、不同地域AS证型分布规律情况以及免疫、影像、系统生物学相关理化指标、组学技术在AS中医辨证客观化领域中的研究进展,意深入从不同的层面阐述AS中医证候的科学内涵,探讨AS中医辨证分型的内在致病机制,以期为中医药辨证治疗AS提供可参考的客观量化指标,为AS中西医结合诊疗模式的发展及临床诊治提供新的研究思路和方法。Ankylosing spondylitis(AS)is a kind of chronic inflammatory autoimmune disease with concealed onset,which belongs to“Dalou”in traditional Chinese medicine.As the disease progresses,it can be accompanied by various degrees of multi-system damage such as skin and mucosa,eyes,lungs,gastrointestinal tract,kidney systems.Due to the continuous changes in the living environment and lifestyle,the incidence of AS has shown an upward trend in recent years,and the syndrome differentiation and treatment of AS in traditional Chinese medicine has obvious advantages in alleviating inflammation,reducing pain,and improving joint function.AS is one of the dominant diseases treated by traditional Chinese medicine.This article reviewed the research progress of standardization and objectification of syndrome differentiation of AS,mainly summarized the standardization of syndrome differentiation and classification of AS,the distribution of syndrome types of AS in different regions and the research progress of immunity,imaging,system biology related physical and chemical indicators and the histological technology in the field of objectification of syndrome differentiation of AS in traditional Chinese medicine,with the intention of elaborating the scientific connotation of syndrome differentiation of AS from different levels.It explored the internal pathogenic mechanism of syndrome differentiation and classification of AS in order to provide objective quantitative indicators for the treatment of AS with syndrome differentiation and provide new research ideas and methods for the development and clinical diagnosis and treatment of AS with integrated traditional Chinese and western medicine.
