清热解毒消癥汤结合解救治疗对晚期乳腺癌临床疗效、肿瘤标记物、中医证候积分影响研究

摘　　要：目的 探讨清热解毒消癥汤结合解救治疗对乳腺癌患者临床疗效、肿瘤标记物和中医证候积分的影响。方法 选择于医院就诊的151例乳腺癌患者(2019年1月-2022年3月收治)作为研究对象。所有患者随机分为对照组(75例)和治疗组(76例),对照组采用解救治疗,治疗组患者在对照组治疗的基础上,口服清热解毒消癥汤(组方:生山楂15 g,丹参15 g,败酱草15 g,太子参15 g,乳香15 g,车前草20 g,虎杖20 g,没药20 g,白花蛇舌草20 g,麝香12 g,生大黄12 g,当归12 g,菟丝子12 g,炒白术12 g,郁金12 g,炙黄芪30 g,牛黄6 g)治疗。21 d为1个疗程,两组均治疗3个疗程。比较两组治疗前后中医证候积分,比较两组治疗前后肿瘤标记物[糖类抗原(CA153)、糖类抗原(CA125)、癌胚抗原(CEA)]水平变化,比较两组治疗疗效,采用SCL-90量表评价两组患者治疗前后的生存质量、比较两组患者治疗不良反应情况。结果 治疗组治疗总有效率(86.84%,66/76)高于对照组患者(72.00%,54/75)(P<0.05);两组中医证候评分均显著降低,治疗组中医证候评分低于对照组(P<0.05);肿瘤标记物(CA153、CA125、CEA)水平均较治疗前降低,治疗组肿瘤标记物水平低于对照组(P<0.05);治疗组患者胃肠道反应以及骨髓抑制、脱发、肝肾功能异常等不良反应率均低于对照组患者(P<0.05);治疗组治疗后症状自评量表(SCL-90量表)评分高于对照组(P<0.05)。结论 清热解毒消癥汤结合解救治疗可显著改善乳腺癌患者的临床症状,降低肿瘤标记物水平,提高治疗疗效,降低化疗不良反应情况,改善患者的生存质量,值得进一步研究探讨。Objective To explore the effect of Qingre Jiedu Xiaozheng Decoction(清热解毒消癥汤)combined with rescue treatment on clinical efficacy,tumor markers and TCM syndrome score of breast cancer patients.Methods A total of 151 breast cancer patients admitted to the hospital from January 2019 to March 2022 were selected as the research subjects.All patients were randomly divided into control group(75 cases)and treatment group(76 cases).The control group received rescue treatment.On the basis of the treatment in the control group,the patients in the treatment group were treated with Qingre Jiedu Xiaozheng Decoction orally[raw Shanzha(Crataegi Fructus)15 g,Danshen(Salviae Miltiorrhizae Radix Et Rhizoma)15 g,Baijiangcao(Patriniae Herba)15 g,Taizishen(Pseudostellariae Radix)15 g,Ruxiang(Olibanum)15 g,Cheqiancao(Plantaginis Herba)20 g,Huzhang(Polygoni Cuspidati Rhizoma Et Radix)20 g,Moyao(Myrrha)20 g,Baihuasheshecao(Hedyotidis Herba)20 g,Shexiang(Moschus)12 g,raw Dahuang(Rhei Radix Et Rhizoma)12 g,Danggui(Angelicae Sinensis Radix)12 g,Tusizi(Cuscutae Semen)12 g,Yujin(Curcumae Radix)12 g,processed Huangqi(Astragali Radix Praeparata Cum Melle)30 g and Niuhuang(Bovis Calculus)6 g].21 days was a course of treatment,and both groups were treated for 3 courses.The TCM syndrome scores before and after treatment between the two groups,the changes in the levels of tumor markers(CA153,CA125,CEA)before and after treatment and the therapeutic efficacy of the two groups were compared.The quality of life of the two groups before and after treatment was evaluated by using the SCL-90 scale and the adverse reactions of the two groups before and after treatment were compared.Results The total effective rate in treatment group(86.84%,66/76)was higher than that in control group(72.00%,54/75)(P<0.05).The TCM syndrome score of the treatment group was significantly lower than that of the control group(P<0.05).The levels of tumor markers(CA153,CA125 and CEA)in the treatment group were lower than those in the control group(P<0.05).The rate of
