王行宽教授用益肾疏肝化瘀汤治疗胡桃夹综合征经验总结

摘　　要：王行宽教授结合胡桃夹综合征的常见临床表现与自身经验认为,胡桃夹综合征属于中医“肾风”“肾水”范畴,病位在肝肾,与劳累相关。其病机在于肾之藏精功能失调与肝之疏泄功能失司。肾藏精功能离不开肾之精气充盛,肾络通畅;肝疏泄功能依托于肝木之气畅达,肝体荣养。王行宽教授分析上述病机后,总结出“主以补肾益精、辅以和血化瘀、兼以固涩营精、不忘杂病治肝”的治疗总则,将益肾、疏肝、化瘀三法相结合,创制益肾疏肝化瘀汤治疗胡桃夹综合征。此方在参芪地黄汤补肾益气养阴的基础上,加强了补肾养阴、疏肝化瘀的力量。Professor WANG Xingkuan,combined with the common clinical manifestations of nutcracker syndrome and his own experience,believes that nutcracker syndrome belongs to the category of“kidney wind”and“kidney water”in traditional Chinese medicine.The disease is located in the liver and kidney and is related to fatigue.Its pathogenesis lies in the dysfunction of kidney's function of storing essence and the loss of liver's function of discharging.The normal function of the kidney is related to the energy of the kidney itself,and so is the liver.After analyzing the above pathogenesis,Professor WANG Xingkuan summarized the general treatment principle of“mainly tonifying the kidney and invigorating the essence,supplemented by harmonizing the blood to remove blood stasis,and simultaneously solidifying the astringent essence and treating the liver without forgetting miscellaneous diseases”.He combined the three methods of tonifying the kidney,soothing the liver and removing blood stasis to create Yishen Shugan Huayu Decoction(益肾疏肝化瘀汤)for the treatment of nutcracker syndrome.On the basis of Shenqi Dihuang Decoction(参芪地黄汤)for tonifying kidney,supplementing Qi and nourishing Yin,this prescription strengthens the power of tonifying kidney,nourishing Yin,dispersing liver and removing blood stasis.
