从脾论治2型糖尿病研究进展

摘　　要：糖尿病作为一种常见的慢性代谢性疾病,对人类身体健康造成极大威胁,其中2型糖尿病患者占有极大比例。近年来,中医药凭借多靶点、多途径的治疗特点在2型糖尿病的治疗方面不断取得重大突破。通过对古今典籍资料的梳理,得出脾虚贯穿2型糖尿病的始终,脾虚精微不布与脾虚痰浊内生,均与2型糖尿病的发生发展密切相关。自古代就有从脾论治2型糖尿病的理论文献支撑,结合多项研究表明,从脾论治2型糖尿病的中药治疗效果突出,其作用机制可概括为:胰岛素增敏,改善胰岛素抵抗,改善胰岛β细胞功能,改善糖脂代谢,改善氧化应激,抑制炎症反应,调节肠道菌群结构等方面。文章对从脾论治2型糖尿病的研究进展及相关机制进行了综述,以期为中医从脾论治2型糖尿病提供理论依据与指导。As a common chronic metabolic disease,diabetes poses a great threat to human health,among which type 2 diabetes accounts for a large proportion.In recent years,Chinese medicine by virtue of multi-target,multi-path characteristics of the treatment of type 2 diabetes continue to make major breakthroughs.Through combing the ancient and modern books,it is concluded that the spleen deficiency runs through the whole process of type 2 diabetes,the deficiency of spleen essence and the endogenesis of spleen deficiency and phlegm turbidity are closely related to the occurrence and development of type 2 diabetes.Since ancient times,there has been a theoretical literature support for the treatment of type 2 diabetes from the spleen.Combined with a number of studies,it has been shown that the treatment of type 2 diabetes from the spleen has a prominent therapeutic effect.The mechanism of action can be summarized as insulin sensitization,improvement of insulin resistance,improvement of isletβcell function,improvement of glucose and lipid metabolism,improvement of oxidative stress,inhibition of inflammatory response,and regulation of intestinal flora structure.This article reviews the research progress and related mechanism of treating type 2 diabetes from spleen,in order to provide theoretical basis and guidance for treating type 2 diabetes from spleen in TCM.
