基于“滋水涵木”法探讨刘焯教授用文静汤治疗儿童抽动障碍

摘　　要：通过对儿童抽动障碍国内外研究现状的阐述,明确现阶段该病的研究热点及可能的诱发原因。从五脏论治本病的病因病机,试述中医治疗该病的特色及优势。刘焯教授认为精血同源,小儿若肾精不足,则肝血亏虚,肝体失养,肝阳上亢,容易肝风内动而发病;或是肾阴不足,滋生内热,日久生火,伴有脾虚生痰,则痰火互结,内动生风而发病,体现了“脾肾虚为本,肝风标为实”的病机改变,采用“滋肾柔肝,健脾化痰,平肝潜阳,镇肝熄风”的疗法,创立了文静汤,以玄参为君药;白芍、龙骨、牡蛎为臣药;麦冬、茯苓、石菖蒲为佐药;甘草为使药,从君臣佐使层面分析了文静汤的组方意义,体现了辽派中医儿科“清润流派”治疗小儿疾病用药要清轻、滋润的学术思想。Through the elaboration of the research status of tic disorder in children at home and abroad,the research hot spots and possible causes of the disease at this stage were identified.On the cause of the disease from the five Zang organs,to discuss the etiology and pathogenesis of treating this disease,and discuss the characteristics and advantages of traditional Chinese medicine in treating this disease.Professor LIU Zhuo believes that the essence and blood are of the same origin.If a child lacks kidney essence,it will lead to liver blood deficiency,loss of nourishment in the liver,and hyperactivity of liver Yang,which can easily lead to the onset of liver wind internal movement;Or it may be due to deficiency of kidney Yin,which breeds internal heat and generates fire over time.Accompanied by spleen deficiency and phlegm generation,phlegm and fire are intertwined,and internal movement generates wind,resulting in the onset of the disease.This reflects the pathological changes of“spleen and kidney deficiency as the foundation,and liver wind as the standard”,adopting the therapy of“nourishing kidney and softening liver,strengthening spleen and resolving phlegm,calming liver and suppressing Yang,and calming liver and suppressing wind”,Wenjing Decoction(文静汤)was established,with Xuanshen(Scrophulariae Radix)as the monarch medicine;Baishao(Paeoniae Radix Alba),Longgu(Draconis Os),Muli(Ostreae Concha)as the official medicines;Maidong(Ophiopogonis Radix),Fuling(Poria),Shichangpu(Acori Tatarinowii Rhizoma)as adjuvants;Gancao(Glycyrrhizae Radix Et Rhizoma)was used as a medicinal herb,and the significance of the formulation of Wenjing Decoction was analyzed from the perspective of monarchs,ministers,and assistants.This reflects the academic idea of the Liao school of traditional Chinese medicine's“Qingrun school”in treating pediatric diseases that medication should be light and nourishing.
