柴葵清消方配合拖线疗法治疗脓肿期浆细胞性乳腺炎临床研究

摘　　要：目的观察柴葵清消方联合拖线法治疗脓肿期浆细胞性乳腺炎的临床疗效。方法将104例入组患者随机分为治疗组和对照组各52例,治疗组采用柴葵清消方联合拖线法治疗,对照组单用中医清创术配合中药内服外敷,分别观察患者术后3、6、9个月乳房恢复情况,以及术前、痊愈后的乳房美容评分情况。结果治疗后,两组在术后3个月,两组的痊愈率均为0,显效率为86.3%,对照组显效率为26.5%,治疗组明显优于对照组(P<0.05)。两组6个月时痊愈率差异无统计学意义(P>0.05)。但治疗组痊愈及显效的患者达到51例(100%),对照组痊愈及显效患者为40例(81.6%)。在术后9个月,治疗组痊愈37例,占72.5%,对照组痊愈24例,占49%,两组9个月时痊愈率差异有统计学意义(P<0.05)。复发率:在痊愈后的6个月时间内,治疗组共复发0例(0),对照组共复发1例(2%),两组对比差异无统计学意义(P>0.05)。乳房美容评分:两组术前乳房美容评分无统计学意义。痊愈后治疗组乳房美容评分高于对照组,差异具有统计学意义(P<0.05)。结论尽管浆细胞性乳腺炎术后恢复时间较长,但清消法配合拖线疗法相较于传统清创法能更早地促进创面恢复,使术后患者痊愈时间缩短,能更小程度地减少对乳房的损伤,减少痊愈后乳房疤痕,能够更好地保证乳房外形及美观。Objective To observe the clinical efficacy of Chaikui Qingxiao Decoction(柴葵清消方)combined with thread dragging method in the treatment of plasma cell mastitis in abscess stage.Methods 104 patients were randomly divided into treatment group and control group,with 52 cases in each group.The treatment group was treated with Chaikui Qingxiao Decoction combined with thread dragging method.The control group was treated with traditional Chinese medicine debridement combined with internal and external application of traditional Chinese medicine.The breast recovery 3,6 and 9 months after operation,as well as the breast beauty score before and after recovery were observed respectively.Results Three months after operation,the cure rate of the two groups was 0,the effective rate was 86.3%,and the effective rate of the control group was 26.5%.The treatment group was significantly better than the control group(P<0.05).There was no significant difference in the cure rate between the two groups at 6 months(P>0.05).However,51 cases(100%)were cured and effective in the treatment group and 40 cases(81.6%)in the control group.At 9 months after operation,37 cases(72.5%)were cured in the treatment group and 24 cases(49%)in the control group.There was significant difference in the cure rate between the two groups at 9 months(P<0.05).Recurrence rate:within 6 months after recovery,there were 0 cases(0)in the treatment group and 1 case(2%)in the control group.There was no significant difference between the two groups(P>0.05).Breast beauty score:there was no significant difference in preoperative breast beauty score between the two groups.After recovery,the breast beauty score in the treatment group was higher than that in the control group(P<0.05).Conclusion Although the postoperative recovery time of plasma cell mastitis is longer,the elimination method combined with thread dragging method can promote the wound recovery earlier than the traditional debridement method,shorten the recovery time of postoperative patients,reduce the
