摘　　要：子宫脱垂是盆腔器官脱垂的主要表现之一,是一种盆底功能障碍性疾病,该病在绝经后妇女中的发病率较高,对患者的身心健康和生活质量产生严重的负面影响。现代医学治疗该病具有一定的局限性,中医药在改善临床症状和预防复发方面有较大优势。杨震教授临证60余载,认为本病病位在筋膜,与肝有密切关系,基于“肝主敷和”“肝主升发”及“肝主筋”的理论认识肝气虚导致子宫脱垂的病机,并根据临床症候特征分为“筋失所养证”“谋虑失司证”“循经见证”三种类型,确立“补肝益气调敷和,助肝升发滋筋膜”的治疗法则,以自拟方补肝颐气汤为主方,根据不同症候类型加减用药,疗效显著。该文从理论渊源、病因病机、分证论治等方面系统阐述杨震教授从肝气虚论治子宫脱垂的学术思想与经验。附验案1则以资佐证。Uterine prolapse is one of the main manifestations of pelvic organ prolapse,which is a pelvic floor dysfunction disease.The incidence of this disease is high in postmenopausal women,and it has a serious negative impact on the physical and mental health and quality of life of patients.Modern medicine has certain limitations in the treatment of this disease,and traditional Chinese medicine has great advantages in improving clinical symptoms and preventing recurrence.Professor YANG Zhen has more than 60 years of clinical syndrome differentiation experience and believes that the location of this disease is in the tendon,which is closely related to the liver.Professor YANG Zhen understood the pathogenesis of uterine prolapse caused by liver Qi deficiency based on the theories of“liver governing harmonization”and“liver governing ascending”and“liver governing the tendon”,and three syndromes of liver Qi deficiency,“tendon loss of nourishment”“loss the ability to make decisions about things”and“symptom of circulating meridian”were concluded to guide the clinical treatment according to the characteristics of clinical symptoms.The law of“replenishing liver Qi to help harmonization and ascending liver to nourish the tendon”was established in clinical practice.The self-designed Bugan Yiqi Decoction(补肝颐气汤)was used as the main prescription to treat uterine prolapse.At the same time,adjusting the dose according to different symptoms is effective.This article systematically expounds Professor YANG Zhen's academic thought and experience in the treatment of uterine prolapse from liver Qi deficiency from the aspects of theoretical origin,etiology and pathogenesis,and treatment by symptoms.A medical case is attached to support it.
