摘  要:目的 研究老年糖尿病肾病(DKD)患者甘露糖结合凝集素2(MBL2)基因单核苷酸多态性(SNP)与肺部感染易感性的关联。方法 随机选取2020年10月-2022年10月宁夏回族自治区人民医院老年DKD并发肺部感染患者78例(感染组)和DKD未合并感染患者74例(对照组)为样本进行分析,收集两组患者基本资料并采用聚合酶链反应(PCR)技术检测MBL2基因SNP,比较两组检测结果并分析MBL2基因SNP与肺部感染的关系。结果 感染组和对照组rs1800450、rs7095891及rs11003125位点基因型分布满足Hardy-Weinberg平衡定律;感染组MBL2基因rs1800450位点GA和AA基因型、rs7095891位点CT和TT基因型占比均高于对照组(P<0.05);条件Logistic回归分析结果显示,MBL2基因rs1800450 GA+AA基因型和rs7095891 CT+TT基因型均为老年DKD患者肺部感染独立危险因素(P<0.05);感染组血浆MBL2水平低于对照组,且两组rs1800450 GG基因型患者血浆MBL2水平高于GA/AA型,rs7095891 CC基因型患者血浆MBL2水平高于CT/TT基因型(P<0.05)。结论 MBL2基因rs1800450和rs7095891位点多态性可导致老年DKD患者血清MBL水平降低,从而增加肺部感染风险。OBJECTIVE To study the relationship between single nucleotide polymorphism(SNP)of mannose-binding lectin 2(MBL2)gene and susceptibility to lung infection in elderly patients with diabetic kidney disease(DKD).METHODS Total of 78 elderly patients with DKD complicated with lung infection(infected group)and 74 patients with DKD without infection(control group)in People's Hospital of Ningxia Hui Autonomous Region between Oct 2020 and Oct 2022 were randomly selected as samples for analysis.The basic data of the two groups of patients were collected,and polymerase chain reaction(PCR)technology was used to detect the SNP of MBL2 gene.The detection results were compared between the two groups and the relationship between SNP of MBL2 gene and lung infection was analyzed.RESULTS The distribution of genotypes at rs1800450,rs7095891 and rsl1003125 loci in the infected group and control group satisfied the Hardy-Weinberg equilibrium law.The proportions of GA and AA genotypes at rs1800450 locus and CT and TT genotypes at rs7095891 locus of MBL2 gene in the infected group were higher than those in the control group(P<0.05).Conditional logistic regression analysis showed that GA+AA genotype at rs1800450 locus and CT+TT at rs7095891 locus of MBL2 gene were the independent risk factors for lung infection in elderly patients with DKD(P<0.05),The level of plasma MBL2 in the infected group was lower than that in the control group,and the plasma MBL2 level in patients with GG genotype at rs1800450 locus was higher than that in patients with GA/AA genotype;the plasma MBL2 level in patients with CC genotype at rs7095891 locus was higher than that in patients with CT/TT genotype(P<0.05).CONCLUSION Polymorphisms of rs1800450 and rs7095891 loci of MBL2 gene can reduce the level of serum MBL level in elderly patients with DKD,thereby increasing the risk of lung infection.
